封闭负压引流联合Masquelet技术对胫骨创伤性骨髓炎所致骨缺损患者临床疗效、骨愈合及生活质量的影响

Effects of Closed Negative Pressure Drainage Combined with Masquelet Technique on Clinical Efficacy,Bone Healing and Quality of Life in Patients with Bone Defect Caused by Tibial Traumatic Osteomyelitis

目的研究封闭负压引流联合Masquelet技术对胫骨创伤性骨髓炎所致骨缺损患者临床疗效、骨愈合及生活质量的影响。方法选取2018年3月-2021年3月收治的胫骨创伤性骨髓炎所致骨缺损88例,按治疗方法的不同分为研究组45例和对照组43例,研究组给予封闭负压引流联合Masquelet技术修复治疗,对照组给予封闭负压引流联合自体骨移植治疗。比较2组临床疗效、骨愈合相关指标、生活质量调查表(SF-36)评分及术后并发症发生情况。结果研究组总有效率显著高于对照组,骨愈合时间、总住院时间及完全负重时间均短于对照组(P<0.05,P<0.01)。术后2组SF-36评分均较术前升高,且术后3个月及6个月研究组SF-36评分明显高于对照组(P<0.05,P<0.01)。研究组术后并发症总发生率明显低于对照组(P<0.05)。结论封闭负压引流联合Masquelet技术对胫骨创伤性骨髓炎所致骨缺损患者的疗效确切,可有效缩短住院时间,促进术后骨愈合,且能有效改善术后生活质量及减少术后并发症的发生。

Objective To study the effect of closed negative pressure drainage combined with Masquelet technique on clinical efficacy,bone healing and quality of life in patients with bone defect caused by tibial traumatic osteomyelitis(TTO).Methods A total of 88 cases of bone defects caused by TTO who were admitted from March 2018 to March 2021 were selected and divided into the research group(n=45)and the control group(n=43)according to different treatment methods.The research group was given closed negative pressure drainage combined with Masquelet technique repair treatment,and the control group received closed negative pressure drainage combined with autologous bone transplantation.The clinical efficacy,bone healing-related indicators,36-item short-form health survey questionnaire(SF-36)scores and postoperative complications were compared between the two groups.Results The total effective rate of the research group was significantly higher than that of the control group,and the bone healing time,total length of hospital stay and full weight-bearing time were all shorter than those of the control group(P<0.05,P<0.01).The SF-36 scores of both groups after operation were higher than those before operation,and the SF-36 scores of the research group were significantly higher than those of the control group at 3 and 6 months after operation(P<0.05,P<0.01).The total incidence of postoperative complications in the research group was significantly lower than that in the control group(P<0.05).Conclusion Closed negative pressure drainage combined with Masquelet technique is effective in treating patients with bone defects caused by TTO,which can effectively shorten the length of hospitalization of patients,promote postoperative bone healing,and can effectively improve the quality of life after surgery and reduce the occurrence of postoperative complications.
