重度支气管哮喘支气管热成形术后行吸气肌训练护理对哮喘控制水平、气道重塑及生活质量的影响

Effect of inspiratory muscle training nursing after bronchial thermoplasty on asthma control level,airway remodeling and quality of life in severe asthma

目的探讨重度支气管哮喘支气管热成形术后行吸气肌训练护理对哮喘控制水平、气道重塑及生活质量的影响。方法选取2018年2月至2020年1月在该院接受治疗的重度支气管哮喘患者96例, 随机分为对照组和观察组, 每组48例。对照组行支气管热成形术治疗后进行常规护理, 观察组在此基础上加以吸气肌训练。比较两组患者临床疗效、哮喘控制水平、气道重塑及生活质量改善情况。结果治疗后观察组总有效率高于对照组, 差异有统计学意义(P<0.05);治疗后两组患者ACT评分及成人哮喘患者生活质量量表得分均明显升高, 但观察组升高幅度明显大于对照组, 差异有统计学意义(P<0.05);治疗后两组患者支气管总管壁厚度、气管内壁厚度及气道平滑肌厚度均明显下降, 但观察组下降幅度明显大于对照组, 差异有统计学意义(P<0.05)。结论支气管热成形术后患者行吸气肌训练可明显提高重度支气管哮喘疗效, 提高患者哮喘控制水平及生活质量, 降低气道重塑水平。

Objective To explore the effect of inspiratory muscle training nursing after bronchial thermoplasty on asthma control level,airway remodeling and quality of life in severe asthma.MethodsA total of 96 patients with severe bronchial asthma who were treated in the hospital from February 2018 to January 2020 were enrolled.They were randomly divided into control group and observation group,48 cases in each group.The control group was given routine nursing after bronchial thermoplasty,while the observation group was given inspiratory muscle training on this basis.The clinical eficacy,asthma control level,airway remodeling and quality of life were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the ACT score and the scores of the Quality of Life in Adults with Asthma scale increased significantly in the two groups,but the increase in the observation group was signifcantly greater than that in the control group,and the dfferences were statistically significant(P<0.05).After treatment,the thickness of the total bronchial wall,the thickness of the inner wall of the trachea and the thickness of the airway smooth muscle in the two groups decreased significantly,but the reduction in the observation group was significantly greater than that in the control group,and the differences were statistically significant(P<0.05).Conclusions Inspiratory muscle training can significantly improve the curative effect of patients with severe bronchial asthma,improve the level of asthma control and quality of life,and reduce the level of airway remodeling.
