Therapeutic effects of different surgical approaches on cryptorchidism in children

目的 探讨不同手术方式治疗儿童不同位置隐睾的疗效。方法 搜集2018年6月-2020年6月在我院小儿泌尿外科收治的216例隐睾患儿的临床资料,年龄10个月~12岁(中位年龄3岁)。根据睾丸位置及不同手术入路分为微创手术组(137例)和传统手术组(79例),其中微创手术入路中阴囊入口处隐睾(74例)经阴囊切口行手术治疗,腹腔型隐睾(63例)行腹腔镜下睾丸固定术。结果 微创手术组的住院时间短于传统手术组,差异有统计学意义(P=0.031);此外,两组均无睾丸萎缩、睾丸回缩的病例,两组并发腹股沟疝、鞘膜积液、伤口愈合不良的发生率及阴囊水肿的发生率、性激素水平比较,差异均无统计学意义(P>0.05)。结论 微创手术与传统手术一样安全有效,但微创手术住院时间短,有利于开展日间手术,还可以为患儿提供良好的美观效果。

Objective To investigate the curative effects of different surgical methods in the treatment of cryptorchidism in children.Methods The clinical data of 216 children with cryptorchidism treated in our hospital during Jun. 2018 and Jun. 2020 were retrospectively analyzed.The patients age ranged from 10 months to 12 years(median 3 years).According to testicular location and different surgical approaches, the patients were divided into minimally invasive surgery group(n=137) and traditional surgery group(n=79).In the minimally invasive surgery group, 74 cases of scrotal cryptorchidism received scrotal incision, and 63 cases of peritoneal cryptorchidism received laparoscopic orchiopexy.Results The minimally invasive surgery group had significantly shorter hospital stay than the traditional surgery group(P=0.031).No testicular atrophy or testicular retraction occurred.There were no differences in the incidence of inguinal hernia, hydrocele, poor wound healing, scrotal edema, and the level of sex hormones between the two groups(P>0.05).Conclusion Minimally invasive surgery is as safe and effective as traditional surgery, but it needs shorter hospital stay, which is conducive to day surgery and cosmetic results.
