三级公立医院文化期望量表的构建及信效度检验

摘要 目的通过德尔菲法筛选编制三级公立医院文化期望与发展现状调查问卷,选取江苏省某地级市三级甲等综合医院医院职工进行实证研究,检验调查问卷的实用性及适用性。方法①本文运用回顾文献、头脑风暴、辩证分析及进行半结构式访谈等方法对量表条目初步制定,两轮德尔菲共邀请11名专家进行专家咨询,对产生的结果利用“界值法”进行分析,“界值法”中包括计算专家的积极系数、权威系数和协调系数等指标得到修改结果,利用Kendall′s W检验结果,根据检验出的结果筛选修改调查问卷条目从而形成初量表。②选取江苏省某地级市三级甲等综合医院360名医院职工对初量表进行预调查,并对结果进行信效度分析。结果两轮专家函询发放的专家咨询表均全部收回,通过计算得出专家权威程度均>0.7,重要性和可操作性的W值均在0.5的范围内波动,通过项目分析及探索性因素分析,该问卷各维度信度均>0.8,KMO统计值均>0.5,表明量表的信效度较好。结论本次调查函询结果可靠,专家的协调程度较好,并最终构建包括9个一级指标和36个二级指标的新时代三级公立医院文化期望与发展调查问卷。 Objective A questionnaire on cultural expectations and development status of tertiary public hospitals was screened and compiled by the Delphi method,and the hospital employees of a tertiary general hospital in a prefecture-level city in Jiangsu Province were selected for empirical research to test the practicability and applicability of the questionnaire.Methods 1.This paper uses the methods of reviewing literature,brainstorming,dialectical analysis and conducting semi-structured interviews to initially formulate the items of the scale.In the two rounds of Delphi,a total of 11 experts were invited for expert consultation,and the results were analyzed using the"boundary value method".The"Boundary Value Method"includes the calculation of experts'positive coefficient,authority coefficient and coordination coefficient and other indicators to get the revised results.Using Kendall's W test results,the questionnaire items were screened and modified according to the test results to form the initial scale.2.Selected 360 hospital staff from a tertiary-level general hospital in a prefecture-level city in Jiangsu Province to conduct a preliminary survey on the initial scale,and analyzed the reliability and validity of the results.Results All the expert consultation forms issued by the two rounds of expert letter inquiries were all recovered, and the degree of authority of experts was calculated to be >0.7. The W value of importance and operability fluctuates within the range of 0.5. Through item analysis and exploratory factor analysis, the reliability of each dimension of the questionnaire was >0.8. The KMO statistic values were all >0.5, indicating that the scale had good reliability and validity. Conclusion The results of the survey letter inquiries are reliable, and the coordination degree of experts is good. Finally, a questionnaire on cultural expectations and development of tertiary public hospitals in the new era, including 9 first-level indicators and 36 second-level indicators, was constructed.
