帕金森病轻度认知障碍患者功能磁共振大尺度脑网络分析 被引量:2

Analysis on large scale resting brain networks in Parkinson's disease with and without mild cognitive impairment

摘要 目的 利用静息状态功能磁共振观察帕金森病认知功能正常(PDNC)及轻度认知障碍患者(PDCI)大尺度神经认知功能网络的改变,探索PDCI发生、发展的脑活动潜在机制。方法 共收集39例PDNC患者、26例PDCI患者及33例健康对照,进行神经心理学评估及静息状态功能磁共振成像扫描。分别利用默认网络、执行控制网络、注意网络和凸显网络4个制式网络模板提取个体水平的脑网络构造,经组间水平统计分析,观察组间水平各网络的连接性变化。结果 PDNC患者的四个制式网络功能连接性减低,而在PDCI呈现了功能连接性增高的变化,而不是进一步的功能连接性减低;执行控制网络则在PDNC阶段已经呈现局部功能连接性增高。结论 帕金森病大尺度神经认知相关网络均发生了类似趋势的功能连接性变化,帕金森认知功能正常患者网络功能连接性减低,而帕金森轻度认知障碍的脑网络基本以代偿性功能连接增强为特征。大尺度脑网络分析对于理解PD认知损害的发生、发展的潜在机制有意义。 Objective To observe the changes of large-scale neurocognitive network in patients with normal cognitive function(PDNC)and mild cognitive impairment(PDCI)in Parkinson s disease by resting state functional magnetic resonance,and to explore the potential brain activity mechanism of the occurrence and development of PDCI.Methods A total of 39 PDNC patients,26 PDCI patients and 33 healthy controls were collected and performed neuropsychological evaluation and resting state functional magnetic resonance imaging scans.Four standard network templates(default network,executive control network,attentional network and salience network)were used to extract the brain network structure at the individual level,and the connectivity changes of each network at the inter-group level were observed through group level statistical analysis.Results The functional connectivity of the four systems in PDNC patients decreased,but generally increased connectivity in PDCI was observed rather than decreased further as the PDNC.The executive control network showed an increase of local functional connectivity in PDNC stage.Conclusions Changes of the functional connectivity in those large-scale neurocognitive related networks in Parkinson s disease with and without mild cognitive impairment has a similar trend.Large-scale brain network analysis is significant for understanding the underlying mechanism of the occurrence and development of PD cognitive impairment.
