Th17在AS患者外周血中的表达及意义

Expression and significance of Th17 in peripheral blood of patients with AS

目的:分析强直性脊柱炎(AS)患者外周血辅助性T细胞17(Th17)的表达及在疾病发生机制中的作用。方法:选择绵阳市中心医院2021年1月至12月收治的AS患者30例为研究组,选择同一时期于该医院进行体检的健康受试者30例为对照组。检测两组受试者Th17、Th1、Th2、人类白细胞抗原B27(HLA-B27)、C反应蛋白(CRP)及红细胞沉降率(ESR)的表达水平,比较两组受试者Th17、Th1、Th2的表达水平,并分析研究组Th17的表达水平与HLA-B27、CRP、ESR之间的相关性。结果:研究组Th17的表达水平明显高于对照组,差异有统计学意义(P<0.05)。研究组Th1的表达水平明显低于对照组,Th2的表达水平明显高于对照组,差异有统计学意义(P<0.05)。研究组HLA-B27的检测结果为阳性,ESR为(32.54±2.24)mm/h,CRP为(31±9)mg/L。经Spearman分析发现:研究组Th17的表达水平与HLA-B27百分比、HLA-B27平均荧光强度及ESR、CRP无明显相关性(r=0.415、r=0.151、r=0.086、r=0.289,P均>0.05),但Th17的表达水平会随着HLA-B27百分比及HLA-B27平均荧光强度的增加而升高。结论:AS患者外周血中Th17、Th2数目增多,Th1数目减少,其体内Th细胞存在失衡情况,提示Th17表达水平的变化是反映AS患者整个免疫异常的重要参考数据。

Objective:To analyze the expression of T helper cell 17(Th17)in peripheral blood of patients with ankylosing spondylitis(AS)and its role in the pathogenesis of the disease.Methods:Thirty patients with AS admitted to Mianyang Central Hospital from January to December 2021 were selected AS the research group,and 30 healthy subjects who underwent physical examination in the hospital during the same period were selected AS the control group.The expression levels of Th17,Th1,Th2,human leukocyte antigen B27(HLA-B27),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)in the two groups were detected,and the expression levels of Th17,Th1 and Th2 were compared between the two groups.The correlation between the expression level of Th17 and HLA-B27,CRP and ESR was analyzed.Results:The expression level of Th17 in study group was significantly higher than that in control group,and the difference was statistically significant(P<0.05).The expression level of Th1 in the study group was significantly lower than that in the control group,and the expression level of Th2 in the study group was significantly higher than that in the control group,and the differences were statistically significant(P<0.05).In the study group,the detection results of HLA-B27 were positive,ESR was(32.54±2.24)mm/h,CRP was(31±9)mg/L.Spearman analysis showed that:There was no significant correlation between the expression level of Th17 and the percentage of HLA-B27,the average fluorescence intensity of HLA-B27,ESR and CRP(r=0.415,r=0.151,r=0.086,r=0.289,all P>0.05).However,the expression level of Th17 increased with the increase of the percentage of HLA-B27 and the average fluorescence intensity of HLA-B27.Conclusion:The number of Th17 and Th2 in peripheral blood of patients with AS is increased,and t he number of Th1 is decreased.There is an imbalance of Th cell s in the body of patients with AS,suggest ing that the change of Th17 expression level is an important reference d ata reflecting the whole immune abnormality of patients with AS.
