BiPAP无创通气联合阿托伐他汀钙对改善老年重症慢阻肺合并呼吸衰竭患者动脉血气指标的效果

Effect of BiPAP non-invasive ventilation combined with atorvastatin calci um on improving arterial blood gas indexes in elderly patients with severe COPD complicated with r espiratory failure

目的:探究BiPAP无创通气联合阿托伐他汀钙对改善老年重症慢性阻塞性肺疾病(慢阻肺)合并呼吸衰竭(RF)患者动脉血气指标的效果。方法:以86例老年重症慢阻肺合并RF患者为试验对象,入组时间2020年6月至2021年6月。随机分为两组各43例。其中常模对照组采用常规治疗+阿托伐他汀钙治疗,无创通气组采用BiPAP无创通气+阿托伐他汀钙治疗。比较两组患者的各项临床指标及治疗效果。结果:相较于常模对照组,无创通气组的呼吸功能恢复时间和住院时间均更短,治疗的总有效率更高(P<0.05)。治疗后,相较于常模对照组,无创通气组的动脉血氧分压(PaO_(2))、血液pH均更高,动脉血二氧化碳分压(PaCO_(2))更低(P<0.05)。治疗后,相较于常模对照组,无创通气组的第1 s用力呼气容积(FEV_(1))、用力肺活量(FVC)和FEV_(1)/FVC均更高(P<0.05)。结论:老年重症慢阻肺合并RF患者采用BiPAP无创通气联合阿托伐他汀钙治疗能显著加快恢复的速度,改善血气指标及肺功能,提升临床治疗效果,值得临床推广应用。

Objective:To explore the effect of BiPAP non-invasive ventilation combined with atorvastatin calcium on improving arterial blood gas indexes in elderly patients with severe chronic obstructive pulmonary disease(COPD)complicated with respiratory failure(RF).Methods:A total of 86 elderly patients with severe COPD complicated with RF were enrolled from June 2020 to June 2021.They were randomly divided into two groups with 43 cases in each group.The normal control group was treated with conventional treatment and atorvastatin calcium,while the non-invasive ventilation group was treated with BiPAP and atorvastatin calcium.The clinical indexes and treatment effects of the two groups were compared.Results:Compared with the normal control group,the recovery time of respiratory function and hospital stay in the non-invasive ventilation group were shorter,and the total effective rate was higher(P<0.05).After treatment,compared with the normal control group,the arterial partial pressure of oxygen(PaO_(2))and blood pH were higher,and the arterial partial pressure of carbon dioxide(PaCO_(2))was lower in the non-invasive ventilation group(P<0.05).After treatment,the forced expiratory volume in the first second(FEV_(1)),forced vital capacity(FVC)and FEV_(1)/FVC in the non-invasive ventilation group were higher than those in the normal control group(P<0.05).Conclusion:BiPAP non-invasive ventilation combined with atorvastatin calcium in elderly patients with severe COPD complicated with RF can significantly accelerate the recovery speed,improve blood gas indexes and lung function,and improve the cl inical treatment effect,which is worthy of clinical applicatio n.
