沙美特罗替卡松配合BIPAP呼吸机治疗慢阻肺合并Ⅱ型呼吸衰竭的效果分析

Efficacy analysis of salmeterol and fluticasone combined with BIPAP venti lator in the treatment of chronic obstructive pulmonary disease with typeⅱrespiratory failure

目的:观察沙美特罗替卡松配合BIPAP呼吸机治疗慢性阻塞性肺疾病(慢阻肺)合并Ⅱ型呼吸衰竭(RF)的效果。方法:选取绵阳市中心医院收治的94例慢阻肺合并Ⅱ型RF患者进行研究,入组时间为2019年8月至2021年12月。随机将其分为两组,其中常模组和讨论组均由47例患者组成。给予常模组BIPAP呼吸机治疗,给予讨论组沙美特罗替卡松配合BIPAP呼吸机治疗。观察并比较两组患者治疗后的肺功能指标、血气指标及运动耐力。结果:治疗后,讨论组的第1 s用力呼气容积(FEV_(1))、用力肺活量(FVC)和FEV_(1)/FVC均明显高于常模组,差异有统计学意义(P<0.05)。治疗后,讨论组的动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))、6 min步行距离(6MWD)均明显优于常模组,动脉血二氧化碳分压(PaCO_(2))明显低于常模组,差异有统计学意义(P<0.05)。结论:慢阻肺合并Ⅱ型RF患者采用沙美特罗替卡松配合BIPAP呼吸机治疗能有效改善呼吸功能和血气指标,提升运动耐力。

Objective:To observe the effect of salmeterol and fluticasone combined with BIPAP ventilator in the treatment of chronic obstructive pulmonary disease(COPD)complicated with typeⅱrespiratory failure(RF).Methods:A total of 94 patients with COPD complicated with typeⅱRF admitted to Mianyang Central Hospital were selected for this study.The enrollment time was from August 2019 to December 2021.All patients were randomly divided into two groups,and 47 patients were included in the normal group and discussion group.The normal group was treated with BIPAP ventilator,and the discussion group was treated with salmeterol fluticasone combined with BIPAP ventilator.The pulmonary function index,blood gas index and exercise endurance were observed and compared between the two groups after treatment.Results:After treatment,the forced expiratory volume(FEV_(1)),forced vital capacity(FVC)and FEV_(1)/FVC in the discussion group were significantly higher than those in the normal group,and the differences were statistically significant(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2)),oxygen saturation(SaO_(2))and 6 min walk distance(6MWD)in the discussion group were significantly better than those in the normal group,and the arterial partial pressure of carbon dioxide(PaCO_(2))was significantly lower than that in the normal group,and the differences were statistically significant(P<0.05).Conclusion:Salmeterol and fluticasone combined with BIPAP ventilator can effectively improve respiratory function and blood gas indexes,and improve exercise endurance in COPD patients with t ypeⅱRF.
