超声造影谐波成像联合彩色多普勒超声对肛瘘的诊断价值

The diagnostic value of contrast-enhanced ultrasound harmonic imaging combined with color Doppler ultrasound in anal fistula

目的:分析超声造影谐波成像联合彩色多普勒超声对肛瘘的诊断价值。方法:选取2020年1月至2022年3月因肛周病变来保定市第一中心医院就诊的疑似肛瘘患者133例为研究对象,术后病理结果确诊为肛瘘患者100例,非肛瘘患者33例。所有研究对象均行超声造影谐波成像、彩色多普勒超声检查。结果:与超声造影谐波成像、彩色多普勒超声单项诊断相比,两者联合检测对肛瘘分型诊断准确率升高(P<0.05)。与超声造影谐波成像、彩色多普勒超声单项诊断相比,两者联合检测对肛瘘内口、脓腔、支管、瘘管诊断准确率升高(P<0.05)。超声造影谐波成像、彩色多普勒超声诊断肛瘘的灵敏度分别为71.00%、73.00%,特异度分别为84.85%、81.82%,准确率分别为74.44%、81.30%,均低于两者联合诊断的97.00%、87.88%、94.74%(均P<0.05)。结论:超声造影谐波成像联合彩色多普勒超声可提高肛瘘分型及肛瘘内口、脓腔、支管的诊断准确率,提升临床对肛瘘的诊断效能。

Objective:To analyze the diagnostic value of contrast-enhanced ultrasound harmonic imaging combined with color Doppler ultrasound in anal fistula.Methods:A total of 133 patients with suspected anal fistula who came to Baoding First Central Hospital from January 2020 to March 2022 were selected as subjects.100 patients were diagnosed with anal fistula and 33 patients were diagnosed with non-anal fistula by postoperative pathology.All subjects were examined by contrast-enhanced harmonic imaging and color Doppler ultrasound.Results:Compared with contrast-enhanced harmonic imaging and color Doppler ultrasound,the diagnostic accuracy of combined detection of anal fistula was higher(P<0.05).Compared with contrast-enhanced harmonic imaging and color Doppler ultrasound,the accuracy of combined detection of internal orifice,purulent cavity,branch and fistula of anal fistula was higher(P<0.05).The sensitivity of contrast-enhanced harmonic imaging and color Doppler ultrasound in the diagnosis of anal fistula was 71.00%and 73.00%respectively,the specificity was84.85%and 81.82%respectively,and the accuracy was 74.44%and 81.30%respectively,which were lower than97.00%,87.88%and 94.74%of the two combined diagnosis(all P<0.05).Conclusion:Contrast-enhanced harmonic imaging combined with color Doppler ultrasound can improve the diagnostic accuracy of anal fistula classification,internal orifice,purulent cavity and branch of anal fistula,and improve the clinical diagnostic efficiency of anal fistula.
