摘要
目的利用人体测量指标包括体重指数(BMI)和血脂参数脂质累积产物(LAP),心脏代谢指数(CMI),评估其对代谢综合征(MetS)的预测能力并获得最佳截断值。方法2021年4月在中国医科大学附属第一医院对已收集的4613名中国东北农村女性资料进行横断面研究,根据绝经与否进行分组。采用2009年亚洲特有标准诊断MetS,利用ROC曲线分析各指标对于代谢综合征的判别能力。结果研究中MetS的患病率为41.62%,LAP和CMI的由线下面积(AUC)值在已绝经女性中为0.88、0.87,在未绝经女性中为0.86、0.85,效果优于BMI。各项指标的最佳截断值在已绝经女性中具有较大值。结论LAP和CMI是预测绝经前后女性代谢综合征的有效指标,最佳截断值在绝经前后存在差别,这将使农村女性MetS的预防和控制更具有针对性和有效性。
Objective To take advantage of anthropometric indicators including body mass index(BMI),and lipid parameters,namely,lipid accumulation product(LAP)and cardiometabolic index(CMI)to evaluate their ability to predict metabolic syndrome(MetS)and obtain the best cut-off value.Methods A cross-sectional study of 4613 rural women in Northeast China was conducted in the First Affiliated Hospital of China Medical Umirersity in April 2021 and these women were divided into groups according to menopause or not.The 2009 Asian unique criteria was used for the diagnosis of MetS,and the ROC curve was used to analyze the ability of each index for predicting metabolic syndrome.Results In this study,the prevalence rate of MetS was 41.62%.The AUC values of LAP and CMI were 0.88,0.87 in postmenopausal women and 0.86,0.85 in premenopausal women,whose effect was better than BMI.The best cut-off value of each index had a greater value in postmenopausal women.Conclusion LAP and CMI are effective indicators for predicting metabolic syndrome in pre-and postmenopausal women,and the best cut-off value is different before and after menopause,which will make the prevention and control of MetS in rural women more targeted and effective.
