非小细胞肺癌患者血清中miRNA-214的表达及其与预后的关系

Expression of serum miRNA-214 in patients with non-small cell lung cancer and its relationship with prognosis

目的探讨非小细胞肺癌(NSCLC)患者血清中miRNA-214的表达及其与预后的关系。方法选取2018年1月至2020年6月苏州大学附属太仓医院收治并确诊的NSCLC患者65例,同时选取同期体检中心体检健康者60例作为对照组,使用实时荧光定量PCR检测miRNA-214表达水平,采用受试者工作特征(ROC)曲线分析miRNA-214在NSCLC中的诊断价值,使用Kaplan-Meier法绘制生存曲线,对比miRNA-214高表达组和miRNA-214低表达组生存期。结果NSCLC患者血清中miRNA-214的相对表达量明显高于健康者,差异有统计学意义(P<0.05);miRNA-214的表达在不同年龄、不同肿瘤直径大小、不同组织学类型之间差异无统计学意义(P>0.05),与病理学分级、淋巴结转移、TNM分期、淋巴血管间隙浸润明显相关,差异有统计学意义(P<0.05)。以1.905为临界值,miRNA-214诊断NSCL的ROC曲线面积为0.811(95%CI:0.733~0.887),敏感度为78.46%,特异度为68.33%;miRNA-214低表达组的生存期明显高于miRNA-214高表达组,差异有统计学意义(P<0.05)。结论miRNA-214在NSCLC患者的血清中呈高表达,与病理学分级、淋巴结转移、TNM分期、淋巴血管间隙浸润明显相关,可以作为NSCLC诊断和判断预后的一项重要指标。

Objective To investigate the expression of serum miRNA-214 in patients with non-small cell lung cancer and its prognostic significance.Methods A total of 65 patients with NSCLC treated and diagnosed in Taicang Hospital Affiliated of Suzhou University January 2018to June 2020 were selected,and 60 healthy subjects in the physical examination center in the same period were selected as the control group.The expression level of miRNA-214 was detected by real-time fluorescence quantitative PCR.The diagnostic value of miRNA-214 in NSCLC was analyzed by receiver operating characteristic(ROC)curve,and the survival curve was drawn by Kaplan Meier method,The survival time of miRNA-214 high expression group and miRNA-214 low expression group were compared.Results The relative expression of miRNA-214 in serum of patients with NSCLC was significantly higher than that of healthy subjects,the difference was statistically significant(P<0.05).The expression of miRNA-214 had no statistically significant difference with different ages,tumor diameters and histological types(P>0.05),but was significantly correlated with pathological grade,lymph node metastasis,TNM stage and lymphatic vascular space infiltration(P<0.05).The ROC curve area(AUC)of miRNA-214 was 0.811(95%CI:0.733-0.887).When the cutoff value was 1.905,the sensitivity was 78.46%and the specificity was 68.33%;The survival time of miRNA-214 low expression group was significantly higher than that of miRNA-214 high expression group(P<0.05).Conclusion miRNA-214 is highly expressed in the serum of patients with NSCLC,which is significantly correlated with pathological grade,lymph node metastasis,TNM stage and lymphatic vascular space infiltration.It can be used as an important index for the diagnosis and prognosis of NSCLC.
