改良尿管拔除法在神经外科留置尿管患者中的效果研究

Study on effect of modified catheter extraction in neurosurgical patients with indwelling catheter

目的 探讨神经外科留置尿管患者使用改良尿管拔除法的效果。方法 93例神经外科留置尿管患者,依据治疗方式不同分为实验组(47例)与对照组(46例)。实验组应用改良尿管拔除法,对照组应用常规尿管拔除法。比较两组患者的尿潴留发生率、舒适度、不同时间的肉眼血尿与隐血试验阳性发生率。结果 两组尿潴留发生率比较差异无统计学意义(P>0.05);实验组舒适度高于对照组,差异有统计学意义(P<0.05)。首次排尿时,实验组的肉眼血尿发生率2.13%、隐血试验阳性发生率57.45%均低于对照组的13.04%、78.26%,差异有统计学意义(P<0.05);拔管24 h后,实验组的肉眼血尿发生率0、隐血试验阳性发生率21.28%均低于对照组的8.70%、69.57%,差异有统计学意义(P<0.05)。结论 改良尿管拔除法对神经外科留置尿管患者效果更好,患者舒适度更高,建议临床参考使用。

Objective To discuss the effect of modified catheter extraction in neurosurgical patients with indwelling catheter.Methods A total of 93 patients with indwelling catheter in neurosurgery were divided into experimental group(47 cases)and control group(46 cases)according to different treatment methods.The experimental group received the modified catheter extraction,while the control group received the conventional catheter extraction.The incidence of urinary retention,comfort level,and incidence of gross hematuria and positive occult blood test at different time points were compared between the two groups.Results There was no statistically significant difference in the incidence of urinary retention between the two groups(P>0.05).The comfort level of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).At the first urination,the gross hematuria rate was 2.13%,and the positive rate of occult blood test was 57.45%in the experimental group,which were lower than 13.04%and 78.26%in the control group,and the differences were statistically significant(P<0.05).After 24 h of extubation,the incidence of gross hematuria was 0 and incidence of positive occult blood test was 21.28%in the experimental group,which were lower than 8.70%and 69.57%in the control group,and the differences were statistically significant(P<0.05).Conclusion The modified catheter extraction has better effect on patients with indwelling catheter in neurosurgery,and the patient's comfort is high,which is recommended for clinical reference.
