1例宫颈碰撞癌患者的临床报告分析

Clinical report analysis of a case with cervical collision cancer

目的探讨宫颈碰撞癌的病理特征与可能病因。方法回顾性研究1例宫颈碰撞癌患者的临床病理资料。结果本例患者的肿瘤由小细胞神经内分泌癌、低分化鳞状细胞癌与高分化腺癌所组成。临床检查结果显示,患者癌胚抗原水平与神经元特异性烯醇化酶水平均上升;左侧附件区存在以囊性为主、分隔囊实性占位灶;子宫颈T2WI信号增高,上缘可达患者子宫体下分,未见高信号影确切突破子宫浆膜层;双侧盆壁骼血管走行区、腹股沟区等多处淋巴结显示增多,部分淋巴结显示增大。经手术治疗后,患者多项指标均得到明显改善,并于术后1周转送至肿瘤科进一步接受化疗。结论宫颈碰撞癌在临床上较为罕见,由于该病的诊疗难度较大,因此临床诊断通常需要依赖患者的术后病检,以避免漏诊、误诊的发生,确保患者的预后效果。

Objective To explore the pathological features and possible etiology of cervical collision cancer.Methods The clinicopathological data of 1 case of cervical collision cancer were retrospectively studied.Results The tumor of this patient consisted of small cell neuroendocrine carcinoma,poorly differentiated squamous cell carcinoma and highly differentiated adenocarcinoma.Clinical examination showed that the levels of carcinoembryonic antigen and neuron-specific enolase increased.In the left adnexal area,there was a cystic,separated cystic and solid mass.The cervical T2WI signal was increased,the upper margin could reach the lower part of the uterine body of the patient,and no high signal shadow was found to break through the uterine serosa layer.Bilateral pelvic wall iliac vascular running area,inguinal area and many other lymph nodes showed increased,some lymph nodes showed enlargement.After sulcal treatment,multiple indexes of the patient were significantly improved,and the patient was transferred to the oncology department one day after surgery for further chemotherapy.Conclusion Cervical collision carcinoma is rare in clinical practice.Due to the difficulty of diagnosis and treatment of this disease,the clinical diagnosis usually needs to rely on the postoperative pathological examination of patients to avoid the occurrence of missed diagnosis and misdiagnosis,and ensure their prognosis.
