乳腺三阴性非特殊型浸润性癌自动乳腺全容积成像特点 被引量:4

Automated breast volume scanning characteristics of triple-negative invasive carcinoma of no special type of breast

摘要 目的观察乳腺三阴性非特殊型浸润性癌(IC-NST)自动乳腺全容积成像(ABVS)超声特点。方法回顾性收集经术后病理证实的156例单发IC-NST患者,根据免疫组织化学结果将其分为三阴性(三阴性组)和非三阴性(非三阴性组),对比组间一般资料及ABVS表现。结果156例中,病变位于左乳76例、右乳80例,三阴性30例(30/156,19.23%)、非三阴性126例(126/156,80.77%)。组间患者年龄、肿瘤体积、淋巴结转移差异均无统计学意义(P均>0.05);病理组织学分级差异有统计学意义(P<0.001),病灶ABVS冠状面“汇聚征”及后方回声衰减差异有统计学意义(P均<0.05),而微钙化、边缘、形态、方位及Adler血流分级差异均无统计学意义(P均>0.05)。结论乳腺三阴性IC-NST的ABVS表现有一定特征性,其中乳腺冠状面“汇聚征”及后方回声衰减相比非三阴性IC-NST较为少见。 Objective To observe the automated breast volume scanning(ABVS)characteristics of triple-negative invasive carcinoma of no special type(IC-NST)of breast.Methods Totally 156 patients with single IC-NST confirmed by postoperative pathology were enrolled and divided into triple-negative subtype(triple-negative group)and non-triple-negative subtype(non-triple-negative group)according to immunohistochemical results,and the patients'general data and ABVS manifestations of lesions were compared between groups.Results Among 156 cases,76 lesions located in the left breast and 80 in the right breast,including 30(30/156,19.23%)triple-negative subtype and 126(126/156,80.77%)non-triple-negative subtype.There was no significant difference of patients'age,tumor volume,nor lymph node metastasis between groups(all P>0.05),whereas significant differences of lesions histological grades(P<0.001),also of the"convergence sign"on coronal plane of ABVS and the posterior echogenic attenuation were found between groups(both P<0.05).Meanwhile,no significant difference of microcalcification,margin,shape,orientation nor Adler blood flow grade of lesions was detected between groups(all P>0.05).Conclusion ABVS features of triple-negative IC-NST had some characteristics.The"convergence sign"on coronal plane ABVS and the posterior echogenic attenuation were relatively rare in triple-negative IC-NST compared with those in non-triple-negative IC-NST.
