不同程度软骨损伤的痛风性膝关节炎关节镜清理术的疗效比较

Comparison of the efficacy of arthroscopic debridement for gouty knee arthritis with different degree of cartilage injury

目的探讨关节镜清理术在不同程度软骨损伤的膝关节痛风性关节炎治疗中的作用。方法回顾性分析2013年1月~2017年12月哈尔滨医科大学附属第四医院骨科36例痛风性膝关节炎患者的临床资料。将患者分为4组,其中1级软骨损伤组7例,2级软骨损伤组8例,3级软骨损伤组10例,4级软骨损伤组11例。对比4组患者治疗前后的疼痛模拟评分法(visualanegensia scale,VAS)评分和Lysholm关节功能评分,评估患者治疗前后的评分差异。结果治疗后4组VAS评分均低于治疗前,Lysholm关节功能评分均高于治疗前,差异有统计学意义(P<0.05)。结论关节镜清理术用于治疗膝关节痛风性关节炎,短期内能有效缓解疼痛、改善关节功能,可作为药物治疗的有效补充手段。

Objective To investigate the role of arthroscopic debridement in treatment of gouty knee arthritis with different degree of cartilage injury.Methods The clinical data of 36 patients with gouty knee arthritis in Department of Orthopedics of the Fourth Affiliated Hospital of Harbin Medical University from January 2013 to February 2017 were retrospectively analyzed.It was divided into grade 1 cartilage injury group(n=7),grade 2 cartilage injury group(n=8),grade 3 cartilage injury group(n=10)and grade 4 cartilage injury group(n=11).Visualanegensia scale(VAS)score and Lysholm joint function score of the four groups were compared before and after treatment.Results After treatment,the VAS scores of the four groups were lower than those before treatment,and the Lysholm joint function scores were higher than those before treatment,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic debridement can effectively relieve pain and improve joint function in the short term,and can be used as an effective supplement to drug therapy.
