低强度激光治疗对正畸引起的牙根吸收的影响

摘要 目的探讨低强度激光治疗对正畸引起的牙根吸收的影响。方法选择2019年3月~2020年2月在本院进行治疗的76例由于正畸治疗引起的牙根吸收患者,随机分为激光组(低强度激光治疗,38例)和安慰剂组(未激活激光装置,38例)。比较两组患者不同时刻第一磨牙腭根近中面根吸收百分比。比较两组患者治疗前后不同部位牙根吸收情况及单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)、细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)水平。结果两组患者第一磨牙腭根近中面牙根吸收百分比组间、时间及交互比较,差异均有统计学意义(P<0.05);与治疗前比较,治疗1周后、治疗2周后激光组牙根吸收百分比均降低(P<0.05),与治疗1周后比较,治疗2周后激光组牙根吸收百分比降低(P<0.05),激光组治疗1周后、治疗2周后牙根吸收百分比均低于安慰剂组(P<0.05)。治疗后激光组上颌及下颌中切牙、侧切牙、尖牙牙根吸收长度均低于治疗前(P<0.05),治疗后激光组上颌及下颌中切牙、侧切牙、尖牙牙根吸收长度均低于安慰剂组(P<0.05)。治疗前两组患者MCP-1、ICAM-1水平比较,差异均无统计学意义(P>0.05),治疗后激光组MCP-1、ICAM-1水平均低于治疗前(P<0.05),治疗后激光组MCP-1、ICAM-1水平均低于安慰剂组(P<0.05)。结论低强度激光治疗对正畸引起的牙根吸收可降低牙根吸收百分比、牙根吸收长度,缓解炎症反应。 Objective To investigate the effect of low-intensity laser treatment on root resorption caused by orthodontics.Methods A total of 76 patients with root resorption caused by orthodontic treatment in our hospital from March 2019 to February 2020 were selected and randomly divided into laser group(low-intensity laser treatment,38 cases)and placebo group(laser device not activated,38 cases).The resorption percentages of the distal buccal and mesial palatal roots of the first molars were compared between the two groups of patients at different times.The root resorption of different parts,the levels of monocyte chemoattractant protein-1(MCP-1)and intercellular adhesion molecule-1(ICAM-1)before and after treatment were compared between the two groups of patients.Results The difference in the percentage of root resorption in the distal buccal side of the first molar and the mesial palatal root between the two groups of patients was statistically significant(P<0.05).Compared with before treatment,the percentage of root resorption in the laser group decreased after 1 week of treatment and after 2 weeks of treatment(P<0.05).Compared with that after 1 week of treatment,the percentage of root resorption in the laser group decreased after 2 weeks of treatment(P<0.05).The percentage of root resorption in the laser group after 2 weeks was lower than that in the placebo group after 1 week of treatment(P<0.05).After treatment,length of root the maxillary and mandibular central incisors,lateral incisors and canines in the laser group were all lower than before treatment(P<0.05).After treatment,resorption of maxillary and mandibular central incisors,lateral incisors and canines in the laser group were lower than the placebo group(P<0.05).Before treatment,there was no significant difference in the levels of MCP-1 and ICAM-1 between the two groups of patients(P>0.05).After treatment,the levels of MCP-1 and ICAM-1 in the laser group were lower than those before treatment(P<0.05).After treatment,the levels of MCP-1 and ICAM-1 in the laser group were lower than those in the placebo group(P<0.05).Conclusion Low-intensity laser treatment can reduce the percentage of root resorption,the length of root resorption,and alleviate the inflammatory response to the root resorption caused by orthodontics.
