电针联合四妙散治疗急性痛风性关节炎疗效观察

Clinical efficacy of electroacupuncture combined with Simiao Powder for acute gouty arthritis

目的观察电针联合四妙散治疗急性痛风性关节炎的临床疗效。方法选择急性痛风性关节炎湿热下注型患者90例,按照随机数字表法分为电针组、中药组和联合组,各30例。在常规治疗的基础上,电针组采取电针治疗,中药组采取四妙散治疗,联合组采取电针联合四妙散治疗。比较3组治疗前后血尿酸、红细胞沉降率(ESR)、C反应蛋白(CRP)、白细胞计数(WBC)水平及中医证候评分变化,统计比较3组临床疗效和不良反应。结果治疗后3组血尿酸、ESR、CRP、WBC水平及各项中医证候评分和总分均较本组治疗前明显下降(P<0.05),且联合组均显著低于电针组、中药组(P<0.05)。联合组总有效率96.67%(29/30),中药组总有效率80.00%(24/30),电针组总有效率76.67%(23/30),联合组总有效率高于电针组和中药组(P<0.05)。联合组、电针组、中药组不良反应发生率分别为6.67%(2/30)、6.67%(2/30)、3.33%(1/30),3组不良反应发生率两两比较差异均无统计学意义(P>0.05)。结论电针联合四妙散治疗急性痛风性关节炎湿热下注型,可显著降低血尿酸水平,改善临床症状,抑制炎症,提高临床疗效。

Objective To observe the clinical efficacy of electroacupuncture(EA)combined with Simiao Powder for acute gouty arthritis(AGA).Methods A total of 90 AGA patients(damp-heat type)were randomly assigned into EA group,traditional Chinese medicine(TCM)group and combination group,with 30 per group,apart from the conventional therapy,EA group received EA,TCM group received Simiao Powder,and combination group received EA combined with Simiao Powder.The aim was to compare serum uric acid(SUA),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),white blood cell count(WBC),TCM syndrome scores;the curative effect and adverse reactions were assessed.Results After treatment,SUA,ESR,CRP,WBC,TCM syndrome scores in the three groups were significantly decreased(P<0.05),which were notably decreased in combination group compared with EA group and TCM group(P<0.05).The overall effective rate in combination group was better than that in EA group and TCM group(96.67%[29/30]vs 80.00%[24/30],76.67%[23/30],[P<0.05],respectively).The adverse reaction rate in combination group,EA group and TCM group was 6.67%(2/30),6.67%(2/30),3.33%(1/30),respectively,the difference of three groups being not significant(P>0.05).Conclusion For AGA patients(damp-heat type),EA combined with Simiao Powder can significantly reduce SUA,improve clinical symptoms,inhibit inflammation,and improve clinical efficacy.
