医院-社区-家庭联动模式在老年血液透析患者中的应用及效果分析

Effect Analysis of Hospital-Community-Family Linkage Care in Elderly Patients with Hemodialysis

目的探讨医院-社区-家庭联动模式在老年血液透析(HD)患者中的应用及效果。方法选择2020年1月—2021年12月在解放军联勤保障部队第九一〇医院治疗的60例老年血液透析患者为研究对象。随机分为对照组和试验组,每组各30例。对照组给予常规护理,试验组加用医院-社区-家庭联动护理,对比两组患者干预前后的效果。结果干预后,试验组在生活质量、SDS、SAS以及总满意度等方面均优于对照组(p<0.05)。结论医院-社区-家庭联动护理可有效改善患者负性情绪,提高患者生活质量和护理满意度。

Objective To explore the application effect of hospital-community-family linkage care mode in elderly hemodialysis patients.Methods A total of 60 elderly patients receiving hemodialysis in the No.910 Hospital,Joint Logistics Support Force of the Chinese People’s Liberation Army from January 2020 to December 2021 were selected as the research subjects.They were randomly divided into control group(n=30)and experimental group(n=30).The control group was given routine nursing care,and the experimental group was given hospital-community-family linkage care,and the application effects of the two groups before and after the intervention were compared.Results After the intervention,the experimental group was better than the control group in terms of quality of life,SDS,SAS and total satisfaction(p<0.05).Conclusion Hospital-communityfamily linkage care can effectively relieve patients’negative emotions,improve patients’quality of life and satisfaction with care,so it is worthy of clinical application.
