钛人工听骨在慢性化脓性中耳炎患者听骨链重建中的应用效果

Application effects of titanium artificial ossicule in ossicular chain reconstruction in patients with chronic suppurative otitis media

目的:观察钛人工听骨在慢性化脓性中耳炎患者听骨链重建中的应用效果。方法:选取2019年4月至2020年4月佳木斯东风区人民医院收治的100例慢性化脓性中耳炎患者进行前瞻性研究,采用随机数字表法将患者分为对照组和观察组,各50例。两组均行鼓室成形术及听骨链重建,对照组采用自体听骨行听骨链重建术,观察组采用钛人工听骨行听骨链重建术,随访12个月。比较两组术前和术后2、6、12个月听力情况(气导听力、骨导听力、气骨导差),以及术后并发症发生率。结果:观察组术后2、6、12个月气导听力、气骨导差均低于术前和对照组;对照组术后6、12个月气导听力、气骨导差均低于术前,差异有统计学意义(P<0.05);两组术后2、6、12个月骨导听力比较,差异均无统计学意义(P>0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:相较于人工听骨,钛人工听骨应用于慢性化脓性中耳炎患者听骨链重建可更好地改善患者的气导听力、气骨导差。

Objective:To observe application effects of titanium artificial ossicule in ossicular chain reconstruction in patients with chronic suppurative otitis media.Methods:100 patients with chronic suppurative otitis media who were admitted to Jiamusi Dongfeng district people’s hospital from April 2019 to April 2020 were selected for the prospective study,and were divided into control group and observation group by using the random number table method,50 cases in each group.Both groups underwent tympanoplasty and ossicular chain reconstruction.The control group received autologous ossicule for ossicular chain reconstruction,while the observation group received titanium artificial ossicule for ossicular chain reconstruction.They were followed up for 12 months.The hearing conditions(air conduction hearing,bone conduction hearing,air-bone gap)before and at 2,6,and 12 months after the surgery,and the incidence of postoperative complications were compared between the two groups.Results:The air conduction hearing and the air-bone gap of the observation group 2,6,and 12 months after the surgery were lower than those before the surgery,those of the observation group were lower than those of the control group;the air conduction hearing and the air-bone gap of the control group 6 and 12 months after the surgery were lower than those before the surgery;and the differences were statistically significant(P<0.05).There were no significant differences in bone conduction hearing between the two groups 2,6,and 12 months after the surgery(P>0.05).Further,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Compared with artificial ossicles,titanium artificial ossicles can better improve the hearing of the patients with chronic suppurative otitis media with ossicular chain reconstruction.
