胫骨平台骨折患者术后发生膝关节僵硬的影响因素 被引量:2

摘要 目的:探讨胫骨平台骨折患者术后发生膝关节僵硬的影响因素。方法:回顾性分析2020年1月至2021年1月该院收治的60例胫骨平台骨折患者的临床资料,将30例胫骨平台骨折术后发生膝关节僵硬患者设为发生组,30例胫骨平台骨折术后未发生膝关节僵硬患者设为未发生组。收集两组基线资料,分析胫骨平台骨折患者术后发生膝关节僵硬的影响因素。结果:发生组合并伸膝装置损伤、合并骨质疏松、合并膝关节周围损伤、术后无规范化康复指导占比均高于未发生组,差异有统计学意义(P<0.05);两组性别、合并高血压、骨折Schatzker分型、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇占比比较,差异均无统计学意义(P>0.05);经Logistic回归分析结果显示,合并伸膝装置损伤、合并骨质疏松、合并膝关节周围损伤、术后无规范化康复指导均为影响胫骨平台骨折患者术后发生膝关节僵硬的危险因素(OR>1,P<0.05)。结论:合并伸膝装置损伤、合并骨质疏松、合并膝关节周围损伤、术后无规范化康复指导均为影响胫骨平台骨折患者术后发生膝关节僵硬的危险因素。 Objective:To explore influencing factors of postoperative knee stiffness in patients with tibial plateau fractures.Methods:The clinical data of 60 patients with tibial plateau fractures admitted to this hospital from January 2020 to January 2021 were retrospectively analyzed.30 patients with knee stiffness after tibial plateau fracture surgery were set as occurrence group,and 30 patients without knee stiffness were set as non-occurrence group.The baseline data of the two groups were collected.The influencing factors of postoperative knee stiffness in patients with tibial plateau fractures were analyzed.Results:The proportions of combined knee extension device injury,combined osteoporosis,combined injury around knee joint,and no standardized rehabilitation guidance after surgery in the occurrence group were higher than those of the non-occurrence group,and the differences were statistically significant(P<0.05).There were no significant differences in gender,combined hypertension,Schatzker classification of fractures,and the ratios of TC,TG,HDL-C,and LDL-C between the two groups(P>0.05).Logistic regression analysis showed that combined knee extensor device injury,combined osteoporosis,combined injury around knee joint,and no standardized postoperative rehabilitation guidance were all risk factors for the postoperative knee stiffness in the patients with tibial plateau fractures(OR>1,P<0.05).Conclusions:Combined knee extensor device injury,combined osteoporosis,combined injury around knee joint,and no standardized postoperative rehabilitation guidance are the risk factors for the postoperative knee stiffness in the patients with tibial plateau fractures.
