血清miR-146 a和H I F-1α水平检测在慢性阻塞性肺疾病诊断中的应用价值 被引量:1

摘要 目的:分析血清miR-146a和缺氧诱导因子1α(HIF-1α)水平检测在慢性阻塞性肺疾病(COPD)诊断中的应用价值。方法:回顾性分析2018年12月至2020年12月该院收治的167例COPD患者和80名健康体检者的临床资料。按照疾病严重程度分为急性加重期组(n=87)和稳定期组(n=80),80名健康体检者为对照组。比较三组血清miR-146a和HIF-1α水平,绘制受试者工作特征(ROC)曲线分析血清miR-146a和HIF-1α水平在急性加重期COPD诊断中的应用价值。结果:急性加重期组和稳定期组的血清miR-146a水平均低于对照组,且急性加重期组低于稳定期组;急性加重期组和稳定期组的HIF-1α水平均高于对照组,且急性加重期组高于稳定期组,差异均有统计学意义(P<0.05);ROC曲线分析结果显示,血清miR-146a和HIF-1α水平单项及联合检测诊断COPD急性加重期的曲线下面积(AUC)分别为0.721、0.744、0.890,均>0.70。结论:血清miR-146a和HIF-1α水平单独及联合诊断COPD患者急性加重期均具有一定价值,且以联合评估价值最佳。 Objective:To analyze application value of serum miR-146a and hypoxia-inducible factor 1α(HIF-1α)level detection in diagnosis of chronic obstructive pulmonary disease(COPD).Methods:The clinical data of 167 COPD patients and 80 healthy subjects admitted to this hospital from December 2018 to December 2020 were retrospectively analyzed.These patients were divided into acute exacerbation group(n=87)and stable phase group(n=80)according to the severity of the disease;while 80 healthy subjects were set as control group.The serum levels of miR-146a and HIF-1αwere compared among the three groups.Receiver operating characteristic(ROC)curve was drawn to analyze the application value of serum miR-146a and HIF-1αlevel detection in the diagnosis of acute exacerbation of COPD.Results:The serum miR-146a levels in the acute exacerbation group and the stable phase group were lower than that in the control group,and that of the acute exacerbation group was lower than that of the stable phase group;the levels of HIF-1αin the acute exacerbation group and the stable phase group were higher than that in the control group,and that of the acute exacerbation group was higher than that of the stable phase group;and the differences were statistically significant(P<0.05).The ROC curve results showed that the areas under the curve(AUC)of the single miR-146a detection,single HIF-1αdetection and combined detection in the diagnosis of acute exacerbation of COPD were 0.721,0.744,and 0.890,respectively(all>0.70).Conclusions:single miR-146a detection,single HIF-1αdetection and combined detection have certain diagnostic value in the diagnosis of the acute exacerbation of COPD patients,and the diagnostic value of the combined evaluation is the highest.
