高危型HPV阴性子宫颈癌患者的临床病理特点分析

Clinicopathological characteristics of high-risk HPV-negative cervical cancer

目的研究高危型人乳头瘤病毒(HPV)阴性子宫颈癌患者的临床病理特点。方法选取68例高危型HPV阴性子宫颈癌患者,所有患者HPV标本进行检测,并按照检测结果分为阳性组(35例)与阴性组(33例)。对比两组患者病理类型及临床分期。结果两组鳞癌、腺鳞癌、小细胞癌病理类型检出率对比,差异无统计学意义(P>0.05),阴性组腺癌病理类型检出率39.39%明显高于阳性组17.14%,差异有统计学意义(P<0.05)。两组Ⅰa期、Ⅰb1期、Ⅰb2期、Ⅱa期临床分期对比,差异无统计学意义(P>0.05),但阴性组Ⅱb期、Ⅲ~Ⅳ期临床分期均高于阳性组,差异有统计学意义(P<0.05)。结论高危型HPV阴性子宫颈癌中阴性患者的病理类型主要为腺癌,临床分期特点主要为Ⅱb期、Ⅲ~Ⅳ期,在临床诊断中需要综合考虑患者病理类型与临床分期特点,从而提升疾病诊断准确率。

Objective To investigate the clinicopathological characteristics of high-risk human papillomavirus(HPV)-negative cervical cancer.Methods 68 patients with high-risk HPV-negative cervical cancer were selected.All the patients'HPV samples were tested and divided into positive group(35 cases)and negative group(33 cases)according to the test results.The pathological types and clinical stages of the two groups were compared.Results There was no significant difference in the detection rate of pathological types of squamous cell carcinoma,adenosquamous cell carcinoma and small cell carcinoma between the two groups(P>0.05),The detection rate of pathological types of adenocarcinoma in the negative group was 39.39% significantly higher than that in the positive group(17.14%),and the difference was statistically significant(P<0.05).There was no significant difference in the clinical stages of stagela,stage Ⅰb1,stage Ⅰb2 and stage Ⅱa between the two groups(P>0.05),but the clinical stages of stage Ⅱb and stage Ⅲ~Ⅳ in the negative group were higher than those in the positive group,and the differences were statistically significant(P<0.05).Conclusion The pathological type of negative patients in high-risk HPV negative cervical cancer is mainly adenocarcinoma,and the clinical stage characteristics are mainly stagellb and stage Ⅲ~Ⅳ.it is necessary to comprehensively consider the pathological type and clinical stage characteristics of patients in clinical diagnosis,so as to improve the accuracy of disease diagnosis.
