探讨宫腔灌注治疗慢性子宫内膜炎对反复种植失败患者妊娠结局的影响

To investigate the effect of intrauterine perfusion in the treatment of chronic endometritis on pregnancy outcome in patients with repeated implant failure

目的分析宫腔灌注治疗慢性子宫内膜炎对反复种植失败(RIF)患者妊娠结局的影响。方法选取86例慢性子宫内膜炎RIF患者,根据随机数字表法分为研究组(采用宫腔灌注治疗)与对照组(采用常规治疗),每组43例。比较两组患者妊娠结局及炎性因子水平。结果研究组患者妊娠率97.67%、活产率79.07%显著高于对照组79.07%、48.84%,差异有统计学意义(P<0.05),两组早期流产率、异位妊娠率及多胎妊娠率比较,差异无统计学意义(P>0.05)。研究组患者肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)及降钙素原(PCT)水平显著低于对照组,干扰素养-γ(IFN-γ)水平显著高于对照组,差异有统计学意义(P<0.05)。结论宫腔灌注治疗对慢性子宫内膜炎RIF患者具有显著疗效,可有效改善患者妊娠结局,提高患者妊娠率,减少患者流产率与早产率,降低患者异位妊娠率,对改善患者预后具有极为重要的作用,值得广泛推广。

Objective To analyze the effect of intrauterine perfusion on pregnancy outcomes in patients with chronic endometritis and repeated implantation failure(RIF).Methods 86 RIF patients with chronic endometritis were selected and divided into study group(intrauterine perfusion therapy)and control group(conventional treatment)according to the random number table method,with 43 cases in each group.Pregnancy outcomes and inflammatory factors were compared between the two groups.Results The pregnancy rate(97.67%)and live birth rate(79.07%)of the study group were significantly higher than those of the control group(79.07% and 48.84%),and the differences were statistically significant(P<0.05).There was no significant difference in the early abortion rate,ectopic pregnancy rate and multiple pregnancy rate between the two groups(P>0.05),The levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and procalcitonin(PCT)in the study group were significantly lower than those in the control group,and the levels of interferon-γ(IFN-γ)in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Intrauterine perfusion therapy has a significant effect on RIF patients with chronic endometritis,which can effectively improve the pregnancy outcome,improve the pregnancy rate,reduce the rate of abortion and premature delivery,and reduce the rate of ectopic pregnancy.It plays an extremely important role in improving the prognosis of patients,and is worthy of widespread promotion.
