不同手术方式治疗卵巢子宫内膜样囊肿疗效及术后康复情况对比分析

Comparative analysis of the curative effect and postoperative rehabilitation of ovarian endometrioid cyst treated by different surgical methods

目的分析不同手术方式治疗卵巢子宫内膜样囊肿患者的临床效果。方法选取110例卵巢子宫内膜样囊肿患者,根据手术方式不同分为A组与B组,各55例。A组采用开腹手术治疗,B组采用腹腔镜手术治疗。对比两组手术指标、2年内与5年内复发率、术后并发症发生情况及性激素水平。结果B组术中出血量少于A组,手术时间、住院时间及术后排气时间均短于A组,差异有统计学意义(P<0.05)。B组2年内复发率与5年内复发率均明显低于A组,差异有统计学意义(P<0.05)。B组并发症发生率3.64%明显低于A组1&18%,差异有统计学意义(P<0.05)。治疗前,两组卵泡刺激素(FSH)、雌二醇(E_(2))及促黄体生成素(LH)水平比较,差异无统计学意义(P>0.05);治疗后1、6个月,B组LH、FSH水平高于A组,E_(2)水平低于A组,差异有统计学意义(P<0.05)。结论相比开腹手术,腹腔镜手术治疗卵巢子宫内膜样囊肿有利于患者术后康复,可缩短患者住院与手术时间,减少术中出血量,降低并发症发生,值得临床推广与应用。

Objective To analyze the clinical effect of different surgical treatment methods on patients with ovarian endometrioid cyst.Methods 110 patients with ovarian endometrioid cyst were selected and divided into group A and group B according to different surgical methods,55 cases in each group.Group A was treated with open surgery,and group B was treated with laparoscopic surgery.The surgical indicators,recurrence rate within 2 years and 5 years,postoperative complications and sex hormone levels were compared between the two groups.Results The intraoperative blood loss in group B was less than that in group A,and the operation time,hospital stay and postoperative exhaust time were shorter than those in group A,and the differences were statistically significant(P<0.05).The recurrence rates within 2 years and 5 years in group B were significantly lower than those in group A,and the differences were statistically significant(P<0.05).The complication rate of group B(3.64%)was significantly lower than that of group A(18.18%),and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of follicle stimulating hormone(FSH),estradiol(E2)and luteinizing hormone(LH)between the two groups(P>0.05).At 1 and 6 months after treatment,the levels of LH and FSH in group B were higher than those in group A,and the levels of E2 were lower than those in group A,and the differences were statistically significant(P<0.05).Conclusion Compared with open surgery,laparoscopic surgery for the treatment of ovarian endometrioid cyst is beneficial to the postoperative rehabilitation of patients,can shorten the hospitalization and operation time,reduce intraoperative blood loss,reduce the incidence of complications,which is worthy of clinical promotion and application.
