铜绿假单胞菌注射液联合顺铂治疗恶性胸水的临床研究

Clinical study of pseudomonas aeruginosa injection combined with cisplatin in the treatment of malignant pleural effusion

目的 探讨铜绿假单胞菌注射液联合顺铂治疗恶性胸水的临床疗效和安全性。方法 选择茂名市中医院2018年1月-2021年1月就诊的肺癌合并恶性胸水患者,分为联合治疗组和对照组,各66例。2组患者均给予顺铂胸腔灌注治疗,联合治疗组在顺铂治疗的基础上采用铜绿假单胞菌注射液联合治疗。观察、检测2组患者的临床疗效、治疗前后积液中的癌肿瘤标志物含量以及治疗安全性。结果 联合治疗组患者的DCR显著高于对照组(P<0.05)。治疗后,2组患者的胸腔积液中的癌胚抗原(CEA)、鳞状上皮细胞癌相关抗原(SCCAg)、细胞角化蛋白片段19(CYFRA21-1)、可溶性人白细胞抗原-G(ShlA-G)水平均显著降低,且联合治疗组胸腔积液中的CEA、SCCAg、CYFRA21-1、ShlA-G水平低于对照组(P<0.05)。治疗期间,联合治疗组胃肠道反应不良反应发生率低于对照组(P<0.05)。结论 铜绿假单胞菌注射液联合顺铂治疗恶性胸水的临床疗效优于单独顺铂治疗,且有助于降低患者肠胃道不良反应。

Objective To investigate the clinical efficacy and safety of Pseudomonas aeruginosa injection combined with cisplatin in the treatment of malignant pleural effusion.Methods Patients with lung cancer complicated with malignant pleural effusion treated in our hospital from January 2018 to January 2021 were randomly divided into combined treatment group and control group,with 66 cases in each group.Both groups of patients were given cisplatin perfusion therapy,The combined treatment group was treated with Pseudomonas aeruginosa injection on the basis of cisplatin treatment.Observe and detect the clinical efficacy of the two groups of patients,the levels of cancer and tumor markers in the effusion before and after treatment,and the safety of treatment.Results The DCR of the combined treatment group was significantly higher than that of the control group(P<0.05).After treatment,the levels of CEA,SCCAg,CYFRA21-1,and ShlA-G in the pleural effusion of the two groups were significantly reduced,and the levels of CEA,SCCAg,CYFRA21-1,and ShlA-G in the pleural effusion of the combined treatment group Lower than the control group(P<0.05).During the treatment,the incidence of gastrointestinal reactions in the combined treatment group was lower than that in the control group(P<0.05).Conclusion The clinical efficacy of Pseudomonas aeruginosa injection combined with cisplatin in the treatment of malignant pleural effusion is better than that of cisplatin alone,and it is helpful to reduce gastrointestinal adverse reactions.
