不同程度锁骨短缩的有限元分析

Finite element analysis of different degrees of clavicular shortening

摘要 目的分析不同外展载荷下锁骨短缩的生物力学特性,为临床研究锁骨短缩提供有效的理论依据。方法选取1名健康成年男性志愿者,基于断层CT扫描的影像数据,通过软件处理并建立包括锁骨、肩胛骨、韧带组织的有限元模型。模拟锁骨短缩程度与人体上臂外展载荷,分析不同外展载荷、不同锁骨短缩程度下有限元模型中各部位的最大应力分布情况。结果60°、90°、120°外展载荷下,锁骨、肩胛骨、喙肩韧带、肩锁韧带、斜方韧带、锥状韧带的最大应力随着锁骨短缩程度的增加而增大;喙肩韧带、肩锁韧带、斜方韧带、锥状韧带在不同外展载荷、不同锁骨短缩程度下,均为肩锁韧带承受的最大应力值最大,喙肩韧带承受的最大应力值最小。结论锁骨短缩较为复杂,不同外展载荷下,肩关节骨骼、韧带最大应力随锁骨短缩程度的增加而增大,对于因锁骨短缩导致出现严重症状者应通过手术恢复锁骨长度。 ObjectiveTo analyze the biomechanical characteristics of clavicular shortening under different abduction loads,so as to provide an effective theoretical basis for clinical study of clavicular shortening.MethodsA healthy adult male volunteer was selected.The finite element model including clavicle,scapula and ligament tissue was processed and established through software based on the image data of CT scanning.Different degrees of clavicular shortening and the abduction load of human upper arm were simulated,and the maximum stress distribution in each part of the finite element model under different abduction loads and different degrees of clavicular shortening was analyzed.ResultsThe maximum stress of clavicle,scapula,coracoacromial ligament,acromioclavicular ligament,trapezoidal ligament and conical ligament increased with the increase of clavicle shortening under 60°,90°,120°abduction load.Between coracoacromial ligament,acromioclavicular ligament,trapezoidal ligament,conical ligament,the acromioclavicular ligament supported the maximum stress,and the coracoacromial ligament supported the minimum maximum stress under different abduction loads and different degrees of clavicular shortening.Conclusion Clavicular shortening is complex.The maximum stress of shoulder bones and ligaments increases with the increase of clavicular shortening under different abduction loads.The length of clavicle should be restored by surgery for those with serious symptoms caused by clavicular shortening.
