老年帕金森病患者血清MCP-1、MIP-1α水平的表达及对临床预后的影响

Expression of serum MCP-1 and MIP-1α levels in elderly patients with Parkinson’s disease and its influence on clinical prognosis

目的 分析血清巨噬细胞炎性蛋白-1α(MIP-1α)、单核细胞趋化蛋白-1 (MCP-1)水平对老年帕金森病(PD)患者预后的影响。方法 选取2019年1月至2020年1月在武汉市第三医院就诊的80例老年PD患者作为研究对象,所有患者均经药物治疗,于治疗1年时评估两组预后[蒙特利尔认知评估量表(MoCA)评分]情况,并分为不良组(MoCA评分<26分)与良好组(MoCA评分≥26分),设计基线资料统计表,详细统计两组患者的基线资料,重点分析治疗前血清MCP-1、MIP-1α对老年PD患者预后的影响。结果 治疗1年时,80例老年PD患者中有38例MoCA评分<26分,占比47.50%(38/80),纳入不良组;不良组血清MCP-1、MIP-1α水平比良好组高(P<0.05);经回归分析结果显示,血清MCP-1、MIP-1α过表达是预后不良危险因子(OR>1,P<0.05);绘制ROC曲线,结果显示,老年PD患者治疗前血清MCP-1、MIP-1α水平预测预后不良风险AUC均>0.80,预测价值均理想,且以联合预测价值最高。结论 血清MCP-1、MIP-1α异常过表达可能与老年PD患者预后不良有关。

Objective To analyze the influence of serum macrophage inflammatory protein-1 α(MIP-1 α)and monocyte chemoattractant protein-1(MCP-1) levels on the prognosis of elderly patients with Parkinson’s disease(PD).Methods 80 elderly patients with PD admitted to Wuhan Third Hospital from January 2019 to January 2020 were selected as the research subjects,all patients were treated with drugs,the prognosis [Montreal Cognitive Assessment(MoCA) score] of the two groups at 1 year of treatment was evaluated,and divided them into a bad group(MoCA score <26 points) and a good group(MoCA score≥26 points),the statistical table of baseline data was designed,the baseline data of patients in the two groups were counted in detail,the influence of serum MCP-1 and MIP-1 α before treatment on the prognosis of elderly patients with PD was mainly analyzed.Results One year after treatment,of 80 elderly patients with PD,38 had MOCA score <26 points,accounting for 47.50%(38/80),included in the bad group;serum MCP-1 and MIP-1α levels in the poor group were higher than the good group(P<0.05),the results of Logistic regression showed that the over expression of serum MCP-1,MIP-1 α were risk factors for poor prognosis(OR>1,P<0.05);the ROC curve was drawn,and the results showed that,the AUC of serum MCP-1 and MIP-1 α levels before treatment in predicting the risk of poor prognosis in elderly patients with PD were>0.80,the predictive value was ideal,and the combined predictive value was the highest.Conclusion The abnormal overexpression of serum MCP-1 and MIP-1 α levels may be related to poor prognosis in elderly PD patients.
