摘要
目的 分析HIV合并丙型肝炎病毒(hepatitis C virus,HCV)感染患者的CD4^(+)T细胞及肝功能状态,为HIV/HCV诊疗提供科学依据。方法 本研究选取2019-01/2020-12期间云南省传染病医院收治的HIV/HCV合并感染患者126例,同时随机筛选378例HIV/AIDS患者进行配对研究。收集人口学特征、CD4^(+)T淋巴细胞计数及肝功能指标。采用SPSS19.0软件包进行统计分析,非正态分布的变量资料以中位数和四分位数间距M(IQR)表示,多个样本均数的比较采用秩和检验,以P<0.05为差异有统计学意义。结果 HIV/AIDS患者与HIV/HCV合并感染患者的性别、年龄分布差异没有统计学意义,而在感染途径上差异有统计学意义。HIV/AIDS患者与HIV/HCV合并感染者相比,CD4^(+)T淋巴细胞计数减少更为明显(P<0.05)。HIV/HCV合并感染患者ALT、AST和APRI评分均高于HIV/AIDS患者。结论 HIV/HCV合并感染患者免疫功能低下,加速肝纤维化进程,发展为慢性肝炎及肝硬化失代偿的机率更高。
Objective To analyze the status of CD4^(+)T cells and liver function in HIV patients with HCV co-infection,and to provide scientific basis for the diagnosis and treatment of HIV/HCV.Methods A total of 126 HIV/HCV co-infected patients were selected from January 2019 to December 2020 in Yunnan Provincial Hospital of Infection Disease,and 378 HIV/AIDS patients were randomly selected for a paired study.Demographic characteristics,CD4^(+)T lymphocyte count and liver function indexes of all patients were collected.SPSS 19.0 software package was used for statistical analysis.Non-normally distributed variable data were expressed as median and interquartile spacing M(IQR).Rank-sum test was used to compare the mean values of multiple samples,and P<0.05 was considered statistically significant.Results There were no significant differences in gender and age between HIV/AIDS patients and HIV/HCV co-infected patients,but there were statistically significant differences in infection route.CD4^(+)T lymphocyte count decreased more significantly in HIV/AIDS patients than that in HIV/HCV co-infected patients(P<0.05).ALT,AST and APRI scores in HIV/HCV co-infected patients were higher than those in HIV/AIDS patients.Conclusions HIV/HCV co-infection patients with low immune function and accelerated liver fibrosis process,chronic hepatitis and cirrhosis decompensated more likely.
