正常成人踝关节相关放射学参数的测量及临床意义 被引量:3

Measurement and clinical significance of ankle-related radiological parameters in normal adults

摘要 目的通过对正常成人踝关节正侧位X射线片的相关形态学参数进行测量分析,为踝关节骨折及韧带损伤的诊断和治疗提供参考。方法选取我院120例正常成人的踝关节正侧位X射线片,运用PACS图文分析系统分别测量内踝高度(H1)、踝内侧间隙(L1)、胫距上关节间隙(L2)、胫踝角(A1)、内踝倾斜角(A2)、踝穴弧长(L3)、侧位踝穴角(A3)、侧位踝穴高度(H2)共8组数据,并比较男性和女性的差异。结果男性和女性的踝内侧间隙(L1)、胫踝角(A1)、内踝倾斜角(A2)测量值比较差异无统计学意义(P>0.05),而内踝高度(H1)、胫距上关节间隙(L2)、踝穴弧长(L3)、侧位踝穴角(A3)、侧位踝穴高度(H2)测量值比较差异有统计学意义(P<0.05)。结论根据踝关节正侧位X射线片相关影像学参数范围,可以为踝关节骨折及韧带损伤的诊断与治疗提供帮助;内踝高度(H1)、胫距上关节间隙(L2)、踝穴弧长(L3)、侧位踝穴角(A3)、侧位踝穴高度(H2)存在性别差异,在应用其作为参考指标时,需考虑性别因素。 Objective To provide a reference for the diagnosis and treatment of ankle fracture and ligament injury through the measurement and analysis of the related morphological parameters of the X-ray of the normal adult ankle.Methods The anteroposterior and lateral X-ray of the ankle of 120 normal adults in our hospital were selected.Eight groups of data including medial malleolus height(H1),medial clear space(L1),superior clear space(L2),tibial ankle angle(A1),medial malleolus inclination angle(A2),tibial axc length(L3),lateral mortise angle(A3)and lateral mortise height(H2)were measured by PACS graphic analysis system,and the differences between male and female were compared.Results There was no statistically significant difference in the measured values of medial clear space(L1),tibial ankle angle(A1)or medial malleolus inclination angle(A2)between male and female(P>0.05),while there were statistically significant differences in the measured values of medial malleolus height(H1),superior clear space(L2),tibial axc length(L3),lateral mortise angle(A3)and lateral mortise height(H2)between male and female(P<0.05).Conclusion According to the range of X-ray related imaging parameters of the anteroposterior and lateral of the ankle,it can provide help for the diagnosis and treatment of ankle fracture and ligament injury.There are gender differences in medial malleolus height(H1),superior clear space(L2),tibial axc length(L3),lateral mortise angle(A3)and lateral mortise height(H2).Gender factors should be considered when applying them as reference indicators.
