靶点穿刺在PVP治疗椎体压缩性骨折中的应用效果分析 被引量:1

摘要 目的:探讨在经皮椎体成形术(PVP)治疗陈旧性骨质疏松性椎体压缩性骨折(OVCF)中采用靶向穿刺的疗效。方法:行PVP治疗的陈旧性OVCF患者65例,根据术中穿刺是否到达靶点分为靶点组42例和对照组23例。比较两组术后骨水泥弥散情况,术后3 d视觉模拟评分(VAS),术后1个月Oswestry功能障碍指数(ODI)及并发症发生率。结果:两组手术时间、出血量、骨水泥注入量及住院时间比较,差异均无统计学意义(P>0.05)。术前靶点组VAS评分和ODI评分分别为7.63±0.52分和76.53±5.63分,对照组分别为7.54±0.23分和74.58±4.25分,差异均无统计学意义(P>0.05);术后靶点组VAS评分和ODI评分分别为2.42±0.24分和32.02±6.32分,对照组分别为2.61±0.34分和36.12±5.21分,术后两组VAS评分和ODI评分较术前降低,且靶点组低于对照组,差异均有统计学意义(P<0.05)。骨水泥弥散在骨折线内靶点组39例(92.86%),对照组15例(65.22%),靶点组骨水泥弥散情况优于对照组,差异具有统计学意义(P<0.05)。靶点组骨水泥渗漏9例(21.43%),对照组无渗漏,靶点组骨水泥渗漏发生率高于对照组,差异具有统计学意义(P<0.05)。结论:经皮PVP中靶点穿刺能提高椎体内骨水泥弥散强化程度,有效缓解疼痛,改善患者脊椎功能和生活质量,但可能增加骨水泥渗漏的风险。
