弱精子症患者精浆果糖与精液质量精子动态参数的相关性研究

Correlation of seminal fructose with semen quality and sperm motility parameters in patients with asthenospermia

目的探讨弱精子症(AS)患者精浆果糖含量与精液质量、精子动态参数之间的相关性。方法选取2021年5月至11月在广州市番禺区中心医院男性科就诊的132例男性为研究对象,根据精子前向运动百分率(PR)将其分为:对照组(79例),轻度弱精子症组(25例),中、重度弱精子症组(28例)。检测所有受试者精液质量、精子正常形态率、精子动态参数、精浆果糖含量,分析精浆果糖含量与精液质量、精子动态参数之间的相关性。结果各组间精液体积、pH值差异无统计学意义(P>0.05)。与对照组相比,中、重度弱精子症患者精子浓度、精子总数、精子摆动性(WOB)、精浆果糖含量降低,差异有统计学意义(P<0.05)。与对照组相比,轻度及中、重度弱精子症患者PR、精子总活力、精子正常形态率、精子曲线速率(VCL)、精子直线速率(VSL)、平均路径速率(VAP)降低,差异有统计学意义(P<0.01),与轻度弱精子症患者相比,中、重度弱精子症患者PR、精子总活力,VCL,VSL,VAP进一步降低,差异有统计学意义(P<0.05)。相关性分析显示,精浆果糖浓度与PR、精子总活力及VAP呈正相关(P<0.05)。结论弱精子症患者的精子表现为运动速度降低、运动方式改变及精浆果糖含量降低,且果糖浓度与精子运动速度呈正相关。

Objective To investigate the correlation of seminal fructose content with semen quality and sperm motility parameters in patients with asthenospermia.Methods A total of 132 men who visited the Andrology Department of Panyu District Central Hospital in Guangzhou between May and November of 2021 were included as the subjects.According to the percentage of forward motion(PR)of the sperm,they were divided into the normal control group(n=79),mild asthenospermia group(n=25),moderate to severe asthenospermia group(n=28).The semen quality,rate of normal sperm morphology,sperm motility parameters and seminal fructose content in all subjects were measured,and the correlation of seminal fructose content with semen quality and sperm motility parameters was determined.Results There were no significant differences in semen volume and p H value among the groups(P>0.05).Compared with the control group,the concentration,total counts,wobble(WOB)and seminal fructose content of sperm in patients with moderate to severe asthenospermia were statistically lower(P<0.05).Compared with the control group,the PR,total motility,rate of normal morphology,curvilinear velocity(VCL),straight-line velocity(VSL),and average path velocity of the sperms in patients with mild or moderate to severe asthenospermia were significantly lower(P<0.01).Compared with mild asthenospermia patients,moderate to severe asthenospermia patients showed even lower PR,total motility,VCL,VSL and VAP with statistically significant differences(P<0.05).Correlation analysis showed that seminal fructose content was positively correlated with PR,total motility and VAP of the sperm(P<0.05).Conclusion Patients with asthenospermia present with lower sperm velocity,altered sperm motility and decreased seminal fructose content of the sperm.There is a positive correlation between the seminal fructose content,sperm motility and velocity.
