电针刺激联合镜像治疗对脑梗死患者下肢运动、步态及生活质量的影响

Influence of electroacupuncture stimulation combined mirror therapy on lower extremity movement,gait and quality of life in patients with cerebral infarction

目的:探究电针刺激联合镜像治疗对脑梗死患者下肢运动功能、步态及生活质量的影响。方法:选择在我院住院治疗的100例脑梗死患者,随机均分为镜像治疗组(接受镜像治疗),联合治疗组(镜像治疗联合电针刺激治疗),治疗2周,采用测力计、改良的Ashworth量表(MAS)、Berg平衡量表(BBS)、脑梗死专用生活质量量表(SS-QOL)以及全身三维步态与运动分析系统(RealGait 2.0)评价两组患者肌力、肌张力、平衡、生活质量以及步态情况。结果:治疗2周后,与镜像治疗组比较,联合治疗组BBS评分[(41.36±6.47)分比(44.42±5.19)分]、步行速度[(57.13±12.28)cm/s比(63.04±11.45)cm/s]、步频[(78.22±10.96)步/min比(85.45±12.32)步/min]、步幅[(78.37±14.97)cm比(86.93±13.31)cm]以及SS-QOL总分[(205.62±18.64)分比(213.66±15.46)分]均显著增加,双支撑时间[(1.03±0.47)s比(0.84±0.46)s]显著缩短(P<0.05或<0.01)。结论:采用电针刺激联合镜像治疗可显著改善脑梗死患者下肢步态、平衡功能及生活质量。

Objective:To explore influence of electroacupuncture stimulation combined mirror therapy(MT)on lower extremity motor function,gait and quality of life in patients with cerebral infarction(CI).Methods:A total of 100 CI patients hospitalized in our hospital were randomly and equally divided into MT group(received MT)and combined treatment group(received MT combined electroacupuncture stimulation).Both groups were treated for two weeks.Dynamometer,modified Ashworth scale(MAS),Berg balance scale(BBS),stroke specific quality of life scale(SS-QOL)and whole body 3D gait and motion analysis system(RealGait 2.0)were used to assess muscle strength,muscle tone,balance,quality of life and gait in two groups.Results:Compared with MT group after two-week treatment,there were significant rise in BBS score[(41.36±6.47)scores vs.(44.42±5.19)scores],walking velocity[(57.13±12.28cm/s)vs.(63.04±11.45cm/s)],step frequency[(78.22±10.96)steps/min vs.(85.45±12.32)steps/min],stride length[(78.37±14.97)cm vs.(86.93±13.31)cm]and SS-QOL total score[(205.62±18.64)scores vs.(213.66±15.46)scores],and significant reduction in dual-support time[(1.03±0.47)s vs.(0.84±0.46)s]in combined treatment group,P<0.05 or<0.01.Conclusion:Electroacupuncture stimulation combined mirror therapy can significantly improve the gait and balance function of lower extremity and quality of life in patients with cerebral infarction.
