“防中柱压缩棒”在胸腰椎骨折后路经皮椎弓根螺钉复位固定中的疗效观察

摘要 目的:探讨“防中柱压缩棒”在胸腰椎骨折后路经皮椎弓根螺钉复位固定中的疗效。方法:选取2020年9月~2021年11月本院收治的胸腰椎骨折患者40例,全部患者均接受后路经皮椎弓根螺钉复位固定治疗,其中,20例患者应用“防中柱压缩棒”实施后路经皮椎弓根螺钉复位内固定(观察组),20例患者经皮椎弓根螺钉传统直棒复位内固定术(对照组),比较两组患者手术时间、出血,治疗前后的伤椎前缘高度与后凸Cobb角及椎管占位改善情况,同时为患者实施随访,了解伤椎高度维持情况及预后情况。结果:相较之下,观察组具备更少出血量和并发症,安全性更高(P<0.05)。结论:“防中柱压缩棒”在胸腰椎骨折后路经皮椎弓根螺钉复位过程会提高复位质量,减少并发症,缩减工具数量和操作过程中的器械相互干扰,减化程序提高效率。可控的轴向撑开恢复伤椎高度,间接复位伤椎椎管占位和伤椎终板塌陷。 Objective:To explore the effect of"anti middle column compression rod"in posterior percutaneous pedicle screw reduction and fixation of thoracolumbar fracture.Methods:40 patients with thoracolumbar fractures treated in our hospital from September 2020 to November 2021 were selected.All patients received posterior percutaneous pedicle screw reduction and fixation.Among them,20 patients were treated with posterior percutaneous pedicle screw reduction and internal fixation with"anti middle column compression rod"(observation group)and 20 patients were treated with percutaneous pedicle screw reduction and internal fixation with traditional straight rod(control group).Before and after treatment,the Cobb angle of kyphosis and the space occupation of the spinal canal were improved.At the same time,the patients were followed up to understand the maintenance of the height of the injured vertebra and the prognosis.Results:In contrast,the observation group had less bleeding and complications,and the safety was higher(P<0.05).Conclusion:The"anti middle column compression rod"can improve the reduction quality,reduce the complications,reduce the number of tools and the interference of instruments in the operation process,reduce the procedure and improve the efficiency in the process of posterior percutaneous pedicle screw reduction of thoracolumbar fractures.Controllable axial distraction can restore the height of the injured vertebrae,and indirectly reduce the space occupation of the injured vertebrae and the collapse of the injured vertebrae end plate.
