低剂量薄层CT扫描在肺部肿瘤边界的探究 被引量:1

摘要 目的:观察低剂量薄层CT扫描在不同形态肺部肿瘤边界的应用情况,进而分析比较低剂量薄层CT扫描方式与常规薄层CT扫描方式对肺部肿瘤边界认定准确性。方法:选取2018年8月~2021年8月院内400例疑似肺部肿瘤患者作为研究对象,按肿瘤大致形态分为两组(各200例):观察组病灶形态呈肿块、结节状;对照组病灶形态呈磨玻璃样片状。分别给予两组患者常规剂量薄层CT扫描技术以及低剂量薄层CT扫描技术,图像先采用后处理软件分析,勾画出肿瘤边界,后再经两名高级职称诊断医师分析比较。结果:本文研究显示,观察组患者两者扫描方式所定边界高度吻合192例,占比96.00%,基本吻合7例,吻合度较差1例,差异无统计学意义(P>0.05)。而对照组患者两者扫描方式所定边界高度吻合132例,占比66.00%,基本吻合53例,吻合度较差15例,差异有统计学意义(P<0.05)。结论:借助低剂量薄层CT扫描技术,能够对肿块、结节状此类形态的肺部肿瘤边界精准确定,并且扫描辐射剂量明显减少,应用安全性高,可以完全替代常规薄层CT扫描方式。而对于磨玻璃样片状形态的肺部肿瘤边界认定存在一定差异,应谨慎选择替代常规薄层CT扫描。 Objective:To observe the application of low-dose thin-layer CT scanning in different shapes of lung tumor boundaries,and then analyze and compare the accuracy of low-dose thin-layer CT scanning and conventional thin-layer CT scanning in identifying lung tumor boundaries.Methods:400 patients with suspected lung tumors in the hospital from August 2018 to August 2021 were selected as the study subjects,and were divided into two groups(200 cases each)according to the general shape of the tumors:observation group:the shape of the lesions was mass and nodule;In control group,the shape of lesions was ground glass.Two groups of patients were given conventional dose thin-layer CT scanning technology and low-dose thin-layer CT scanning technology.The images were analyzed by post-processing software to delineate the tumor boundary,and then analyzed and compared by two senior professional diagnostic physicians.Results:This study showed that 192 cases(96.00%)of observation group were highly anastomosed at the boundary defined by the scanning mode,7 cases were basically anastomosed,and 1 case was poorly anastomosed.The difference was not statistically significant(P>0.05).In control group,132 cases(66.00%)were highly anastomosed at the boundary defined by the two scanning modes,53 cases were basically anastomosed,and 15 cases were poorly anastomosed.The difference was statistically significant(P<0.05).Conclusion:With the help of low-dose thin-layer CT scanning technology,the boundary of lung tumors with such shapes as masses and nodules can be accurately determined,and the radiation dose of scanning can be significantly reduced.It is safe to use,and can completely replace the conventional thin-layer CT scanning mode.However,there are some differences in the identification of lung tumor boundaries with ground glass flake shape,and it should be carefully selected to replace conventional thin-layer CT scanning.
