彩超联合数字减影血管造影术指导胆囊穿刺引流术在老年高危胆囊炎治疗中的应用价值

Application Value of Color Doppler Ultrasound Combined with Digital Subtraction Angiography in the Treatment of High-Risk Cholecystitis in the Elderly

目的:分析彩超联合数字减影血管造影术指导胆囊穿刺引流术在老年高危胆囊炎治疗中的应用价值。方法:收集本院2018年7月~2021年6月收治的80例老年高危胆囊炎患者展开分析,以奇偶数字表法分组,记作对比组、研究组,单组例数40例,分别在数字减影血管造影术、彩超联合数字减影血管造影术引导下进行胆囊穿刺引流术,评估治疗效果及价值。结果:与对比组(85.00%)比较,研究组手术成功率(97.50%)更高(P<0.05);与对比组比较,研究组穿刺次数、穿刺透视时间以及总穿刺时间更少,辐射暴露剂量更低(P<0.05);与对比组(20.00%)比较,研究组并发症发生率(5.00%)更低(P<0.05);与对比组比较,研究组QoR-15量表筛选5个维度评分均更高。结论:在老年高危胆囊炎治疗中,采取彩超联合数字减影血管造影术指导胆囊穿刺引流术,手术成功率较高,且能够明显优化临床相关指标,降低并发症发生率,提高术后早期恢复质量,因此,彩超联合数字减影血管造影术指导胆囊穿刺引流术可作为推荐在老年高危胆囊炎治疗中推广、应用。

Objective:To analyze the value of color Doppler ultrasound combined with digital subtraction angiography in the treatment of high-risk cholecystitis in the elderly.Methods:80 elderly patients with high-risk cholecystitis treated in our hospital from July 2018 to June 2021 were collected and analyzed.They were grouped by odd and even number table and recorded as control group and study group.There were 40 cases in a single group.Gallbladder puncture and drainage were performed under the guidance of digital subtraction angiography,color Doppler ultrasound and digital subtraction angiography respectively,and the therapeutic effect and value were evaluated.Results:Compared with the control group(85.00%),the success rate of the study group(97.50%)was higher(P<0.05).Compared with the control group,the puncture times,puncture fluoroscopy time and total puncture time in the study group were less,and the radiation exposure dose was lower(P<0.05).Compared with the control group(20.00%),the complication rate of the study group(5.00%)was lower(P<0.05).Compared with the control group,the QoR-15 scale in the study group scored higher in five dimensions.Conclusion:Conclusion:In the treatment of senile high-risk cholecystitis,the successful rate of gallbladder puncture and drainage guided by color Doppler ultrasound combined with digital subtraction angiography is high,and it can obviously optimize the clinical related indexes,reduce the incidence of complications and improve the quality of early postoperative recovery.Therefore,color Doppler ultrasound combined with digital subtraction angiography guided gallbladder puncture and drainage can be promoted and applied as a recommendation in the treatment of senile high-risk cholecystitis.
