外周血管超声辅助诊断高血压心脏病的应用价值

Application Value of Peripheral Vascular Ultrasound in the Diagnosis of Hypertensive Heart Disease

目的:探讨外周血管超声辅助诊断高血压心脏病的临床应用价值。方法:选择天津市北辰医院2020年1月~2021年5月收治的高血压心脏病患者102例为研究对象,所有患者经病理检查确诊。根据不同检查方法将患者分为超声心动图组(50例)与研究组(52例),超声心动图组采用单纯超声心动图检查,此基础上,研究组采取外周血管超声辅助检查。比较两组患者诊断准确率、漏诊率及误诊率,左心室与左心房肥厚检出率,左室舒张末期内径(LVDd)、左室后壁厚度(PWT)及舒张末室间隔厚度(IVST)等参数。结果:研究组诊断准确率(96.15%)高于超声心动图组(82.00%),差异有统计学意义(P<0.05);两组漏诊率(14.00%,3.85%)、误诊率(4.00%,0)比较差异无统计学意义(P>0.05);研究组左心室肥厚检出率(86.54%)、左心房肥厚检出率(82.69%)均高于超声心动图组(66.00%,60.00%),差异有统计学意义(P<0.05);研究组患者LVDd(46.41±1.85)mm、PWT(10.25±1.01)mm及IVST(11.70±1.27)mm均低于超声心动图组的(47.63±2.16)mm、(10.95±1.28)mm,(12.51±1.56)mm,差异有统计学意义(P<0.05)。结论:外周血管超声辅助检查可有效提高高血压心脏病的诊断准确率,具有重要的临床应用价值。

Objective:To explore the clinical value of peripheral vascular ultrasound in the diagnosis of hypertensive heart disease.Methods:102 patients with hypertensive heart disease treated in Tianjin Beichen hospital from January 2020 to may 2021 were selected as the research objects.All patients were diagnosed by pathological examination.According to different examination methods,102 patients were divided into echocardiography group(50 cases)and study group(52 cases).The echocardiography group was examined by simple echocardiography.On this basis,the study group was examined by peripheral vascular ultrasound.The diagnostic accuracy,missed diagnosis rate and misdiagnosis rate,the detection rate of left ventricular and left atrial hypertrophy,left ventricular end diastolic diameter(LVDD),left ventricular posterior wall thickness(PWT)and end diastolic ventricular septal thickness(IVST)were compared between the two groups.Results:The diagnostic accuracy of the study group(96.15%)was significantly higher than that of the echocardiography group(82.00%),(P<0.05);There was no significant difference in missed diagnosis rate(14.00%,3.85%)and misdiagnosis rate(4.00%,0)between the two groups,(P>0.05);The detection rates of left ventricular hypertrophy(86.54%)and left atrial hypertrophy(82.69%)in the study group were significantly higher than those in the echocardiography group(66.00%,60.00%),the difference was statistically significant(P<0.05);LVDD(46.41±1.85)mm,PWT(10.25±1.01)mm and IVST(11.70±1.27)mm in the study group were significantly lower than those in the echocardiography group(47.63±2.16,10.95±1.28,12.51±1.56)mm,(P<0.05).Conclusion:Peripheral vascular ultrasound can effectively improve the diagnostic accuracy of hypertensive heart disease and has important clinical value.
