胃液病原微生物核酸检测在早产儿早期感染诊断中的价值 被引量:1

摘要 目的 探讨胃液病原微生物核酸检测在早产儿早期感染诊断中的价值。方法 选取早产儿147例,其中感染组92例,非感染组55例。在出生1 h内未开奶前收集胃液和血液;随后进行胃液病原微生物核酸检测和血液病原微生物培养与鉴定,并进行详细记录。结果 胃液病原微生物核酸检测的效果(灵敏度和特异度为63.04%和96.36%)要显著好于血液病原微生物培养(灵敏度和特异度为42.39%和90.09%),表明早产儿胃液病原微生物核酸检测比血液病原微生物培养在早产儿早期病原微生物感染诊断中效果更好。感染组中病原微生物以巨细胞病毒、呼吸道合胞病毒、肺炎链球菌、肺炎克雷伯杆菌和金黄色葡萄球菌为主。结论 胃液病原微生物核酸检测在早产儿早期感染中具有良好的诊断价值。 Objective To investigate the value of nucleic acid detection of pathogenic microorganisms in gastric juice in the diagnosis of preterm infant early infection.Methods A total of 147 premature infants were selected,including 92 in the infection group and 55 in the non-infection group.Gastric juice and blood were collected at 1 hour after birth before drinking milk.Subsequently,nucleic acid detection of pathogenic microorganisms and bacterial culture and identification of gastric juice were carried out,and the results were detailed recorded.Results The detection efficacy of pathogenic microorganism nucleic acid in gastric juice(sensitivity and specificity were 63.04%and 96.36%,respectively)was significantly higher than that of blood pathogenic microorganism culture(sensitivity and specificity were 42.39%and 90.09%,respectively),indicating that the detection of bacterial nucleic acid in gastric juice of premature infants was better than that in blood.Bacterial culture was more effective in the diagnosis of early bacterial infection in preterm infants.The pathogenic microorganisms in the infection group were mainly cytomegalovirus,respiratory syncytial virus,Streptococcus pneumoniae,Klebsiella pneumoniae,and Staphylococcus aureus.Conclusion Nucleic acid detection of pathogenic microorganisms in gastric juice has good diagnostic value in preterm infants early infection.
