清化辟秽汤治疗新型冠状病毒奥密克戎变异株感染“长阳”患者的临床研究

Clinical Study on Qinghua Pihun Decoction in the Treatment of“Long-term Positive”Patients with Novel Coronavirus Omicron Variant Infection

目的观察清化辟秽汤治疗新型冠状病毒奥密克戎变异株感染“长阳”患者的临床疗效并探讨其作用机制。方法104例患者按随机数字表法分为试验组和对照组,最终纳入101例,试验组49例,对照组52例。两组均予中药基础治疗,对照组继续原处方中药基础治疗,试验组停用原处方,改用清化辟秽汤治疗,观察时间持续至患者2次核酸转阴(采样时间间隔24 h)。比较试验前后中医证候量表评分、中医证候有效率、核酸转阴时间和安全性。结果两组治疗后主要症状、次要症状、总得分、咳嗽、咽痛、乏力、鼻塞指标均较治疗前下降(均P<0.05),且试验组治疗后各评分均低于对照组(均P<0.05)。两组治疗后咯痰、嗅觉减退、味觉减退、肌痛、腹泻得分均较治疗前下降(均P<0.05),组间比较差别均不大(均P>0.05)。试验组总有效率93.88%高于对照组的80.77%(P<0.05)。试验组最后一次、倒数第二次核酸平均转阴时间均短于对照组(均P<0.05)。两组在试验期间均未发生不良反应。结论清化辟秽汤可缩短新型冠状病毒奥密克戎变异株感染“长阳”患者核酸转阴时间,并能改善症状。

Objective:To explore the clinical efficacy of Qinghua Pihun Decoction in the treatment of"longterm positive"patients with Novel Coronavirus Omicron variant infection.Methods:A total of 104"long-term positive"patients with Novel Coronavirus Omicron variant were collected and divided into the experimental group and control group by random number table method.101 patients were included ultimately,with 49 in the experimental group and 52 in the control group.Both groups received TCM basic treatment.The control group continued the original TCM basic treatment,and the experimental group stopped the original prescription and switched to Qinghua Pihun Decoction.The observation lasted until the nucleic acid turned negative twice(the sampling interval was 24 hours).The scores of TCM symptom scale,effective rate of TCM symptom,nucleic acid conversion time and safety were compared before and after the experiment.Results:After treatment,the main symptoms,secondary symptoms,total score,cough,sore throat,fatigue,and nasal congestion in the two groups were all decreased compared with those before treatment(all P<0.05),and the scores in the experimental group were lower than those in the control group after treatment(all P<0.05).After treatment,the five symptom scores of expectoration,hyposmia,hypotaste,myalgia and diarrhea in the two groups were all decreased compared with those before treatment(all P<0.05),but there was no significant difference between the two groups after treatment(all P>0.05).The total effective rate in the experimental group was 93.88%,which was higher than 80.77%in the control group(P<0.05).The mean time of negative nucleic acid negative conversion at the last and second last times in the experimental group was shorter than that in the control group(all P<0.05).There were no adverse reactions in either group during the treatment.Conclusion:Qinghua Pihun Decoction can shorten the nucleic acid negative conversion time of"long-term positive"patients with Novel Coronavirus Omicron variant infection,and can improve symptoms.
