家庭支持和家庭韧性对妇科肿瘤术后复发恐惧的影响分析

Influence of family support and family resilience on fear of recurrence after gynecological oncology surgery

目的 了解妇科肿瘤术后家庭支持和家庭韧性水平,并探讨其与疾病不确定感和肿瘤复发恐惧之间的链式中介效应。方法 利用家庭关怀度指数问卷(APGAR)、家庭韧性评估量表(FRAS)、疾病不确定感量表(MUIS)和肿瘤患者恐惧疾病进展简化量表(FoP-Q-SF)对305例妇科肿瘤手术患者的家庭支持、家庭韧性、疾病不确定感和肿瘤复发恐惧现状进行调查,分析家庭支持、家庭韧性、疾病不确定感与肿瘤复发恐惧的关系及其链式中介效应情况,并分析APGAR、FRAS和MUIS评分预测肿瘤复发恐惧价值。结果 全组APGAR评分为(5.87±1.84)分,FRAS评分为(135.64±30.18)分,MUIS评分为(79.06±21.64)分;家庭支持(r=-0.822,P<0.001)、家庭韧性(r=-0.715,P<0.001)与肿瘤复发恐惧呈负相关,疾病不确定感与肿瘤复发恐惧呈正相关(r=0.744,P<0.001);家庭支持与家庭韧性评分呈正相关(r=0.644,P<0.001),家庭支持与疾病不确定感呈负相关(r=-0.696,P<0.001),家庭韧性与疾病不确定感呈负相关(r=-0.643,P<0.001);家庭支持和家庭韧性在妇科肿瘤术后疾病不确定感和肿瘤复发恐惧中起到链式中介效应;APGAR、FRAS联合MUIS评分预测肿瘤复发恐惧的曲线下面积(AUC)最大。结论 家庭支持和家庭韧性与妇科肿瘤术后复发恐惧有关,在疾病不确定感和肿瘤复发恐惧中起到链式中介效应。

Objective To understand the level of family support and family resilience after gynecological oncology surgery,and to explore the chain-mediated mediating effect between their levels,uncertainty of disease and fear of cancer recurrence.Method The Family Caring Index Questionnaire( APGAR),Family Resilience Assessment Scale( FRAS),Morbidity Uncertainty Scale( MUIS) and Fear of Disease Progression in Oncology Patients Simplified Scale( Fo P-Q-SF) were used to investigate the current status of family support,family resilience,morbidity uncertainty and fear of tumor recurrence in 305 gynecologic oncology surgery patients,to analyze the relationship between family support,family resilience,morbidity uncertainty and fear of cancer recurrence and their chain mediated effects,and to analyze the value of APGAR,FRAS and MUIS scores in predicting fear of tumor recurrence.Result The APGAR score for the whole group was(5.87±1.84),FRAS score was(135.64±30.18),and MUIS score was(79.06±21.64);family support(r=-0.822,P<0.001) and family resilience(r=-0.715,P<0.001) were negatively correlated with fear of tumor recurrence,uncertainty of illness was positively correlated with fear of tumor recurrence(r= 0.744,P<0.001);family support was positively correlated with family resilience score(r= 0.644,P<0.001),family support was negatively correlated with disease uncertainty(r=-0.696,P<0.001),and family resilience was negatively correlated with disease uncertainty(r=-0.643,P<0.001);family support and family resilience played a chain mediating effect in postoperative disease uncertainty and fear of tumor recurrence in gynecologic oncology;APGAR and FRAS combined with MUIS score predicted the largest area under the curve( AUC) for fear of tumor recurrence.Conclusion Family support and family resilience are associated with fear of postoperative recurrence in gynecologic oncology and play a chain mediating effect in the sense of disease uncertainty and fear of tumor recurrence.
