医院重症监护病房患者感染耐碳青霉烯类铜绿假单胞菌分布及耐药性分析 被引量:6

Analysis of distribution and drug resistance of carbapenem-resistant Pseudomonas aeruginosa from hospital ICU patients

摘要 目的了解医院重症监护病房(ICU)住院患者感染耐碳青霉烯类铜绿假单胞菌(CRPA)临床分布和耐药情况。方法通过回顾性分析方法,对某医院ICU临床分离CRPA资料进行分析与评估。结果该医院ICU患者感染CRPA中,医院获得感染和社区获得感染分别占44.24%和52.11%。连续5年社区感染CRPA检出率有升高趋势。医院ICU患者CRPA感染部位均以下呼吸道为主。医院和社区感染CRPA对亚胺培南和美罗培南耐药率较高,对多黏菌素B耐药率为0%。结论医院ICU患者CRPA感染率较高,医院获得感染的CRPA对多种抗菌药物的耐药率均高于社区感染,应采取有针对性的防控措施,科学合理选用抗菌药物。 Objective To investigate the clinical distribution and drug resistance of carbapenem-resistant Pseudomonas aeruginosa(CRPA) in hospital intensive care unit(ICU) patients.Methods Through retrospective analysis, the data of clinically isolated CRPA in a hospital ICU were analyzed and evaluated.Results In the ICU patients in this hospital infected with CRPA,hospital-acquired infection and community-acquired infection accounted for 44.24% and 52.11%,respectively.The detection rate of CRPA in community-acquired infection had been increasing for 5 consecutive years.The infection sites of CRPA in hospital ICU patients were mainly the lower respiratory tract.The drug resistance rates of CRPA in hospital-acquired infection and community-acquired infection to imipenem and meropenem were high, and the resistance rate to polymyxin B was 0%.Conclusion The infection rate of CRPA in hospital ICU patients is relatively high, and the resistance rates of CRPA in hospital-acquired infection to multiple antibiotics are higher than those of CRPA in community-acquired infection.Targeted prevention and control measures should be taken and antibiotics should be selected scientifically and rationally.
