影像IVW工作站对头颈CT血管造影后处理的初步研究

摘要 目的:探究影像赛诺威盛(IVW)后处理工作站高级血管分析软件对头颈CT血管造影(CTA)后处理效果。方法:收集医院收治的85例疑似头颈部血管疾病并行头颈CTA检查的患者,对所有患者的CTA检查影像所得原始数据重建0.625 mm层厚、层间距薄层图像。采用影像IVW工作站进行后处理,测量并记录头颈CTA客观指标(主动脉弓、右侧颈总动脉、虹吸部、右侧大脑中动脉CT值)、IVW工作站后处理时间及结果。由两位影像医师采用4分法分别对容积再现(VR)、最大密度投影(MIP)、曲面重建(CPR)及多平面重建(MPR)图像自动处理效果进行主观评估。结果:在85例患者的CTA中主动脉弓、右侧颈总动脉、虹吸部及右侧大脑中动脉CT值分别为(628.88±98.10)HU、(607.38±117.68)HU、(593.58±112.61)HU和(456.82±86.75)HU,后处理自动分析时间平均(63.64±25.50)s。85例患者中,左右椎动脉及颈内动脉4支血管,VR、MIP图像1/4、2/4、3/4和4/4提取结果分别为2例(占2.35%)、6例(占7.06%)、23例(占27.06%)和54例(占63.53%),CPR图像上能自动标记1支、2支、3支和4支血管的结果分别为1例(占1.18%)、7例(占8.24%)、26例(占30.59%)和51例(占60.00%)。两位医师对VR、MIP和CPR图像主观评估具有很好的一致性,Kappa值分别为0.653、0.776和0.778,均能满足诊断需求。结论:IVW工作站能快速对头颈CTA图像进行后处理,生成满足临床诊断的VR、MIP及MPR图像,明显缩短诊断医师后处理时间,减轻其工作负担。 Objective: To investigate the effect of advanced analysis software of blood vessel of image Insight Vision Workstation(IVW) post-processing workstation on the post-processing of head and neck computed tomography angiography(CTA). Methods: 85 patients with suspected head and neck vascular disease who underwent head and neck CTA examination and admitted to hospital were selected. The obtained raw data of CTA examination image of all patients were reconstructed to form 0.625 mm layer thickness and images with interlayer spacing of thin layer. The image IVW workstation was adopted to implement post-processing, and the CTA objective indicators of head and neck(CT values of the aortic arch, right common carotid artery, siphon and right middle cerebral artery), and the postprocessing time and results of IVW workstation were measured and recorded. Two physicians adopted 4-point scale to respectively conduct subjective assessments for the automatic processing effect of the images of volume rendering(VR), maximum intensity projection(MIP) and curve planar reformation(CPR) and multiple plane reconstruction(MPR). Results: The CT values(HU) of the aortic arch, right common carotid artery, siphon and right middle cerebral artery in CTA of 85 patients were(628.88±98.10)HU,(607.38±117.68)HU,(593.58±112.61)HU and(456.82±86.75)HU, respectively. And the average time of automatic analysis of post-processing was(63.64±25.50) s. In the 4 blood vessels included left and right vertebral arteries and internal carotids of 85 patients, the extracted results of 1/4, 2/4,3/4 and 4/4 of VR and MIP images were 2 cases(2.35%), 6 cases(7.06%), 23 cases(27.06%) and 54 cases(63.53%),respectively. And the results of CPR images that could automatically marked 1, 2, 3 and 4 blood vessels were 1case(1.18%), 7 cases(8.24%), 26 cases(30.59%) and 51 cases(60.00%), respectively. There were commendable consistency in subjective assessments between two physicians for VR, MIP and CPR images, and the Kappa values of them were 0.653, 0.776 and 0.778, respectively, and all of them could meet the requirements of diagnosis. Conclusion:The IVW workstation can quickly conduct post-processing for head and neck CTA images, and to generate VR, MIP and CPR images that can meet clinical diagnosis, which can significantly shorten the post-processing time of physicians in diagnosis and relieve their burden.
