自体颗粒脂肪注射移植与眶隔脂肪释放术治疗泪沟畸形的美学效果对比

Cosmetic Effects of Autologous Fat Granule Injection Grafting and Orbital Septum Fat Release in the Treatment of Tear Trough Deformity:a Comparative Study

目的:探究对于泪沟畸形分别采用自体颗粒脂肪注射移植与眶隔脂肪释放术治疗的美学效果差异。方法:将2018年1月-12月到笔者所在医院治疗泪沟畸形的112例患者的相关资料予以回顾性分析,依据患者手术治疗方式将患者分为脂肪移植组(n=90)与脂肪释放组(n=22),两组患者分别接受自体颗粒脂肪注射移植与眶隔脂肪释放术治疗。比较两组患者手术前后泪沟畸形等级量表(TTRS)评分、术后并发症发生率、手术后患者满意度、术后复发率。结果:术后6个月,两组患者TTRS各项评分及总评分较手术前均明显下降,差异有统计学意义(P<0.05),且脂肪释放组TTRS各项评分及总评分更优于脂肪移植组(P<0.05)。脂肪释放组与脂肪移植组患者术后并发症发生率分别为27.27%与17.78%,差异无统计学意义(P>0.05)。脂肪释放组患者美学满意度明显高于脂肪移植组(P<0.05)。术后6个月与12个月,脂肪释放组与脂肪移植组患者复发率比较,差异无统计学意义(P>0.05);术后24个月,脂肪释放组患者复发率明显低于脂肪移植组(P<0.05)。结论:眶隔脂肪释放术治疗泪沟畸形疗效、美学效果优异,远期复发率低,而安全性与自体颗粒脂肪注射移植相当。

Objective The aim of this study was to explore the differences in cosmetic effects of autologous fat granule injection grafting and orbital septum fat release in the treatment of tear trough deformity.Methods The relevant data of 112 patients who came to the hospital for treatment of lacrimal groove deformity from January to December 2018 were retrospectively analyzed.According to the surgical treatment of the patients,the patients were divided into fat transplantation group(n=90)and fat release group(n=22).Two groups of patients received autologous granular fat injection transplantation and orbital septal fat release surgery respectively.The tear groove deformity rating scale(TTRS)scores,postoperative complications,postoperative patient satisfaction,and postoperative recurrence rate were compared between the two groups of patients before and after surgery.Results At 6 months after operation,the TTRS scores and total scores of the two groups of patients were significantly lower than those before the operation,the difference was statistically significant(P<0.05),and the fat release group TTRS scores and total scores were better than fat grafting Group(P<0.05).The incidence of postoperative complications in the fat release group and fat transplantation group was 27.27%and 17.78%,respectively,and the difference was not statistically significant(P>0.05).The cosmetic satisfaction degree of the fat release group was significantly higher than that of the fat transplantation group(P<0.05).At 6 months and 12 months after operation,there was no significant difference in the recurrence rate between the fat release group and the fat transplantation group(P>0.05),24 months after the operation,the recurrence rate of the fat release group was significantly lower than that of fat transplantation group(P<0.05).Conclusion Orbital septum fat release can achieve excellent curative effect and cosmetic effect in the treatment of tear trough deformity,with low long-term recurrence rate.It is equivalent to autologous fat granule injection grafting in safety.
