可溶性ST2在原发性高血压病患者中表达及诊断价值

Expression and diagnostic value of soluble protein ST2 in patients with essential hypertension

目的观察血清中可溶性ST2蛋白(sST2)水平在原发性高血压(EH)患者中变化并探讨与原发性高血压病患者高血压严重程度的关系,明确sST2在EH中的诊断价值。方法入选2018年9月至2020年3月因EH于徐州市中心医院心内科住院治疗的40例患者为研究对象(EH组),对照组为同期来院进行健康体检非高血压病者20例,所有对象均检测血清sST2水平,并记录非服药状态收缩压和舒张压水平,采用Spearman相关分析法分析sST2水平与收缩压及舒张压水平的相互关系,采用受试者工作特征曲线(ROC曲线)分析sST2水平预测高血压的诊断效能。结果EH组收缩压和舒张压水平均高于对照组,差异有统计学差异(P<0.001),EH组患者sST2水平明显高于对照组,两组比较差异有统计学意义(P<0.001),sST2水平随EH患者收缩压及舒张压水平升高而升高,sST2和收缩压及舒张压呈正相关(r=0.649,P<0.001;r=0.652,P<0.001)。sST2水平预测高血压的曲线下面积(AUC)=0.956,血清sST2水平最佳阈值为21 ng/ml时,预测高血压发生的灵敏度与特异度同时达最高,分别为:87.5%、85.0%,约登指数0.725。结论sST2水平在原发性高血压患者中呈现高表达,并随血压的升高而升高,与收缩压和舒张压均成正相关,检测血清sST2有助于高血压的诊断。

Objective To observe the changes of serum soluble protein ST2(sST2)in patients with essential hypertension(EH),discuss the relationship between sST2 and severity of hypertension,and definite diagnostic value of sST2 to EH.Methods EH patients(n=40)were chosen into EH group and non-hypertensive patients(n=20)were chosen into control group from Department of Cardiology,Xuzhou Central Hospital,from Sept.2018 to Mar.2020.The level of serum sST2 was detected and the levels of systolic blood pressure(SBP)and diastolic blood pressure(DBP)were recorded in all patients in non-medication status.The relationship among serum sST2 level,SBP and DBP was analyzed by using Spearman correlation analysis.The diagnostic efficacy of serum sST2 level for predicting hypertension was analyzed by using receiver operating characteristic(ROC)curve.Results The levels of SBP and DBP were all higher in EH group than those in control group(P<0.001),and serum sST2 level was significantly higher in EH group than that in control group(P<0.001).The level of serum sST2 increased with the increase of SBP and DBP,and serum sST2 level was positively correlated to SBP(r=0.649,P<0.001)and DBP(r=0.652,P<0.001).The area under curve(AUC)of serum sST2 level was 0.956 in hypertension prediction,and the best threshold of serum sST2 level was 21 ng/ml.At the same time,the sensitivity(87.5%)and specificity(85.0%)of serum sST2 level in predicting hypertension reached the highest,and Youden index was 0.725.Conclusion The level of serum sST2 shows a higher expression and increased with the increase of blood pressure in EH patients,and it is positively correlated to SBP and DBP.The detection of serum sST2 level is helpful to the diagnosis of hypertension.
