社区老人主观认知下降与情绪状态的相关分析

Correlation analysis between subjective cognitive decline and emotional state of the community elderly

目的探讨主观认知下降(subjective cognitive decline,SCD)的社区老年人特点以及与抑郁情绪之间的相关性。方法在参加社区活动的上海城区老年人中,采用自制主观认知评价问卷、老年抑郁量表(15-item geriatric depression scale,GDS-15)、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)进行评估。结果共纳入SCD组132例和正常对照组41例。与对照组相比,SCD组的年龄(P=0.001)以及糖尿病患病率(P=0.016)更高,受教育年限(P=0.008)更短,同时其GDS-15总分更高[3(1,4)vs 1(0,3),Z=-2.559,P=0.010];SCD发生率和GDS-15总分呈正相关(r=0.195,P=0.010);校正混杂因素后的二元logistics回归分析显示,GDS-15的焦虑维度得分与SCD发生率显著相关(OR 3.437;95%CI 1.021~11.576;P=0.046)。结论SCD老人具有高龄、低学历、高糖尿病患病率的特点,有焦虑症状的社区老人更容易发生SCD。

Objective To explore the characteristics of community elderly with subjective cognitive decline(SCD) and its correlation with depression.Methods The elderly in Shanghai urban areas who participated in community activities were assessed with the Self-Designed Subjective Cognitive Assessment Questionnaire,15-item Geriatric Depression Scale(GDS-15),and Montreal Cognitive Assessment Scale(MoCA).Results A total of 132 cases in SCD group and 41 ones in normal control group were included.Compared with the control group,the SCD group had older age(P=0.001),higher prevalence of diabetes mellitus(P=0.016),shorter years of education(P=0.008),and higher total score of GDS-15 [3(1,4) vs 1(0,3),Z=-2.559,P=0.010].The incidence of SCD was positively correlated with the total score of GDS-15(r=0.195,P=0.010).Binary Logistics regression analysis after adjusting for confounding factors showed that the score of anxiety dimension in GDS-15 was significantly correlated with the incidence of SCD(OR=3.437;95%CI 1.021~ 11.576;P=0.046).Conclusion The elderly with SCD have the characteristics of advanced age,low education level and high prevalence of diabetes.The community elderly with anxiety symptoms are more likely to develop SCD.
