集束化护理在重症肺炎并发呼吸功能衰竭患者重症监护中的效果观察

Observation on the nursing effect of cluster nursing in intensive care of patients with severe pneumonia complicated with respiratory failure

目的:分析集束化护理在重症肺炎并发呼吸功能衰竭患者重症监护中的应用效果。方法:选取2019年1月—2021年12月重症监护室住院治疗的重症肺炎并发呼吸功能衰竭成年患者64例,采用盲选法分为观察组和对照组,每组32例。对照组给予常规护理,观察组在对照组的基础上予以体位护理、气道护理及并发症护理等集束化护理。护理干预前后检测患者动脉血气指标及凝血指标,并记录并发症发生情况。结果:两组患者护理干预前动脉血气指标及凝血指标比较,差异无统计学意义(P>0.05);干预2周后观察组患者血氧饱和度(SaO_(2))与动脉氧分压(PaO_(2))较对照组显著升高,动脉血二氧化碳分压(PaCO_(2))、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)及纤维蛋白原(Fbg)较对照组明显降低,且并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:集束化护理有助于改善重症监护室重症肺炎并发呼吸功能衰竭患者的动脉血气指标及凝血功能,降低并发症发生风险。

Objective:To observe the application value of cluster nursing in intensive care of patients with severe pneumonia complicated with respiratory failure.Methods:A total of 64 adult patients with severe pneumonia complicated with respiratory failure hospitalized in the intensive care unit of hefei second people's hospital from January 2019 to December 2021 were selected and divided into observation group and control group by blind selection method,with 32 cases in each group.Patients in the control group were given routine nursing measures,and patients in the observation group were given body position nursing,airway nursing and complications nursing on the basis of the nursing measures in the control group.Arterial blood gas index and coagulation index were detected before and after nursing intervention,and complications were recorded.Results:There were no significant differences in arterial blood gas and coagulation indexes between the two groups before nursing intervention(P>0.05).Two weeks after intervention,arterial oxygen saturation(PaCO_(2)) and arterial partial pressure of oxygen(PaCO_(2))in observation group were significantly increased,partial pressure of carbon dioxide(PaCO_(2)),prothrombin time(PT),activated partial thromboplastin time(APTT)and Fibrinogen(Fbg)were significantly decreased,and the incidence of complications was lower than that in control group(P<0.05).Conclusion:For patients with severe pneumonia complicated with respiratory failure in intensive care unit,cluster nursing can improve arterial blood gas level and coagulation function,and reduce the risk of complications.
