摘要
目的检测胰岛素样生长因子-1(IGF-1)、G蛋白偶联受体54(GPR54)及胃饥饿素在中枢性性早熟患儿中的水平,分析相关性。方法选取2019年5月—2020年10月台州市立医院收治的81例性早熟患儿为观察组,其中部分性性早熟患儿45例,中枢性性早熟患儿36例;另外选取同期30例正常发育儿童为对照组。应用生化分析仪检测卵泡刺激素(FSH)、雌二醇(E_(2))及黄体生成素(LH)水平,采用酶联免疫吸附法检测IGF-1和胃饥饿素水平,应用荧光定量PCR法检测GPR54水平。IGF-1、GPR54及胃饥饿素之间的相关性采用Pearson相关性分析。结果对照组FSH、E_(2)及LH水平分别为(2.85±0.32)IU/L、(8.71±1.26)pg/ml及(5.31±0.58)IU/L;观察组FSH、E_(2)及LH水平分别为(13.24±2.64)IU/L、(21.82±2.18)pg/ml及(16.74±1.73)IU/L。观察组FSH、E及LH水平均显著高于对照组(t=21.440,21.020,35.370,均P<0.05)。对照组IGF-1、GPR54及胃饥饿素水平分别为(50.32±5.26)nmol/L、(9.75±1.62)ng/ml及(2.51±0.28)ng/L;观察组IGF-1、GPR54及胃饥饿素水平分别为(113.68±10.81)nmol/L、(16.24±2.87)ng/ml及(2.75±0.36)ng/L。观察组IGF-1、GPR54及胃饥饿素水平均显著高于对照组,差异均有统计学意义(t=30.720,11.690,3.297,均P<0.05)。中枢性性早熟患儿IGF-1、GPR54及胃饥饿素水平均显著高于部分性性早熟患儿,差异均有统计学意义(均P<0.05)。IGF-1、GPR54及胃饥饿素联合检测预测中枢性性早熟的曲线下面积、灵敏度分别为0.935、94.80%,均显著高于单一检测。Pearson相关性分析发现:IGF-1与GPR54、胃饥饿素均呈正相关(r=0.599,P=0.001;r=0.750,P=0.001),GPR54与胃饥饿素呈正相关(r=0.631,P=0.001)。结论中枢性性早熟患儿血清IGF-1、GPR54及胃饥饿素水平异常上升,联合检测IGF-1、GPR54及胃饥饿素预测儿童中枢性性早熟的价值较高,IGF-1、GPR54及胃饥饿素可成为预测儿童中枢性性早熟的血清标志物。
Objective To detect the levels of insulin-like growth factor-1(IGF-1),G protein-coupled receptor 54(GPR54),and ghrelin in infants with central precocious puberty,analyze the correlation.Methods From May 2019 to October 2020,81 children with precocious puberty treated in Taizhou Municipal Hospital were selected as observation group,including 45 children with partial precocious puberty and 36 children with central precocious puberty;30 normal children without precocious puberty were selected as control group.Biochemical analyzer was used to detect the levels of follicle-stimulating hormone(FSH),estradiol(E_(2)),and luteinizing hormone(LH).Enzyme-linked immunosorbent assay was used to detect the levels of IGF-1 and ghrelin.Fluorescence quantitative PCR was used to detect the level of GPR54.The correlations of IGF-1,GPR54,and Ghrelin were analyzed by Pearson correlation analysis.Results The levels of FSH,E_(2),and LH in control group were(2.85±0.32)IU/L,(8.71±1.26)pg/ml,and(5.31±0.58)IU/L,respectively.The levels of FSH,E_(2),and LH in observation group were(13.24±2.64)IU/L,(21.82±2.18)pg/ml,and(16.74±1.73)IU/L,respectively.The levels of FSH,E,and LH in observation group were statistically significantly higher than those in control group(t=21.440,21.020,35.370,P<0.05).The levels of IGF-1,GPR54,and ghrelin in control group were(50.32±5.26)nmol/L,(9.75±1.62)ng/ml,and(2.51±0.28)ng/L,respectively.The levels of IGF-1,GPR54,and ghrelin in observation group were(113.68±10.81)nmol/L,(16.24±2.87)ng/ml,and(2.75±0.36)ng/L,respectively.The levels of IGF-1,GPR54,and ghrelin in observation group were statistically significantly higher than those in control group(t=30.720,11.690,3.297,P<0.05).The levels of IGF-1,GPR54,and ghrelin in central precocious puberty group were statistically significantly higher than those in partial precocious puberty group(P<0.05).The area under ROC curve and sensitivity of joint detection of IGF-1,GPR54,and ghrelin in prediction of central precocious puberty were 0.935 and 94.80%,respectively,which were higher than those of single detection.Pearson correlation analysis showed that IGF-1 was positively correlated with GPR54 and ghrelin(r=0.599,P=0.001;r=0.750,P=0.001),GPR54 was positively correlated with ghrelin(r=0.631,P=0.001).Conclusion The levels of serum IGF-1,GPR54,and ghrelin in children with central precocious puberty increase significantly;the predictive value of joint detection of IGF-1,GPR54,and ghrelin for central precocious puberty in children is high;IGF-1,GPR54,and ghrelin can be used as detection indicators of central precocious puberty in children.
