胫骨横向骨搬运术对糖尿病足的治疗效果分析 被引量:4

摘要 目的 研究治疗糖尿病足运用胫骨横向骨搬运术的效果。方法 选取2019年6月—2021年5月于临沂东山医院治疗的93例糖尿病足确诊患者作为观察对象,根据患者治疗方式的不同划分为对照组和观察组,对照组采用皮瓣手术,观察组采用胫骨横向骨搬运术。对比两组患者血管内皮生长因子水平、VAS评分、创面愈合时间及并发症发生率、截肢率。结果 手术前,两组血管内皮生长因子水平、VAS评分,差异无统计学意义(P>0.05);手术后,观察组血管内皮生长因子水平(37.05±6.23)比对照组(24.67±5.25)明显更高,差异有统计学意义(t=9.969,P<0.05),观察组VAS评分(0.35±0.23)分明显低于对照组(1.70±0.25)分,差异有统计学意义(t=26.761,P<0.05)。观察组伤口愈合时间(11.53±0.42)周明显低于对照组(13.31±0.59)周,差异有统计学意义(t=16.998,P<0.05)。治疗后,观察组并发症发生率(3.57%)、截肢率(1.78%)明显低于对照组并发症发生率(18.92%)、截肢率(16.22%),差异有统计学意义(χ~2=3.991、4.754,P<0.05)。结论 糖尿病足中运用胫骨横向骨搬运技术进行治疗,可以提升患者体内的血管内皮生长因子,促进创面愈合,减少并发症、截肢风险,值得在临床推广。 Objective To study the effect of transverse tibial bone transfer in the treatment of diabetic foot.Methods A total of 93 patients with diagnosed diabetic foot who came to Linyi Dongshan Hospital for treatment from June 2019to May 2021 were selected as the observation objects.The patients were divided into control group and observation group according to the different treatment methods.The control group was treated with flap surgery,while the observation group was treated with transverse tibial bone transfer.The levels of vascular endothelial growth factor,VAS score,wound healing time,complication rate,amputation rate.were compared between the two groups.Results Before surgery,there was no statistically significant difference in the level of vascular endothelial growth factor and VAS score between the two groups(P>0.05).After operation,the levels of vascular endothelial growth factor in the observation group(37.05±6.23) were significantly higher than those in the control group(24.67±5.25),and the difference was statistically significant(t=9.969,P<0.05).The VAS score of the observation group(0.35±0.23) points was significantly lower than that of the control group(1.70±0.25) points,and the difference was statistically significant(t=26.761,P<0.05).The wound healing time in the observation group(11.53±0.42) weeks was significantly lower than that in the control group(13.31±0.59) weeks,and the difference was statistically significant(t=16.998,P<0.05).After treatment,the incidence of complications(3.57%) and the rate of amputation(1.78%) in the observation group were also significantly lower than those in the control group(18.92%) and the rate of amputation(16.22%),and the differences were statistically significant(χ~2= 3.991,4.754,P<0.05).Conclusion The use of tibial transverse bone transfer technique in diabetic foot treatment can increase vascular endothelial growth factor in patients,promote wound healing,and reduce the risk of complications and amputation,which is worthy of clinical promotion.
