肥胖伴妊娠期糖尿病应用全程协同护理对患者血糖水平、心理状态及母婴结局的影响

Effects of Whole-course Collaborative Nursing on Obesity and Gestational Diabetes Mellitus on Blood Glucose Level,Psychological State and Maternal and Infant Outcomes

目的 探究肥胖伴妊娠期糖尿病应用全程协同护理的可行性。方法 选择2020年6月—2022年6月来厦门大学附属妇女儿童医院就诊的80例肥胖伴妊娠期糖尿病产妇为研究对象,以数字随机表法将其划分为对照组与观察组,每组40例。对照组产妇实施常规护理,观察组产妇实施全程协同护理,对比两组产妇血糖水平、心理状态以及母婴结局。结果 观察组产妇FPG、1 hPG、2 hPG、HbA1c水平较对照组更加趋近于正常值范围,差异有统计学意义(P<0.05);观察组产妇坚韧、忍受消极情绪、积极接受变化、控制以及精神影响心理状态评分显著高于对照组,差异有统计学意义(P<0.05);观察组母婴出现羊水过多、早产、产后出血、新生儿低血糖不良母婴结局发生率明显低于对照组,差异有统计学意义(P<0.05)。结论 针对肥胖伴妊娠期糖尿病产妇实施两种不同护理模式的对比之下,全程协同护理效果更加理想,可显著调节产妇血糖水平、改善心理状态,保障母婴平安,避免不良妊娠事件发生。

Objective To explore the feasibility of applying whole-process collaborative nursing for obesity with gestational diabetes mellitus.Methods 8 0 obese women with gestational diabetes mellitus who came to Affiliated Women and Children’s Hospital of Xiamen University from June 2020 to June 2022 were selected as the research subjects.They were divided into control group and observation group by random table method with 40 patients in each group.The puerperae in the control group were given routine nursing care,and the puerperium in the observation group was treated with whole-process collaborative nursing.Results Compared with the control group,the levels of FPG,1 hPG,2 hPG,and HbA1c in the observation group were closer to the normal range,the difference was statistically significant(P<0.05);the psychological state scores of maternal tenacity,tolerance of negative emotions,positive acceptance of changes,control and mental influence in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05);the incidence of adverse maternal and infant outcomes such as polyhydramnios,premature birth,postpartum hemorrhage,and neonatal hypoglycemia in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion In the comparison of two different nursing modes for obese women with gestational diabetes mellitus,the effect of wholeprocess collaborative nursing is more ideal.It can significantly regulate maternal blood sugar levels,improve psychological state,ensure the safety of mothers and babies,and avoid adverse pregnancy events.
