探讨尿常规定性检验在诊断糖尿病肾病中的应用价值

Exploring the Application Value of Urine Routine Test in the Diagnosis of Diabetic Nephropathy

目的 探讨尿常规定性检验在诊断糖尿病肾病中的应用价值。方法 以2021年1月—2022年4月在龙岩市第一医院接受治疗的糖尿病肾病患者113例为观察组,再选取同期在本院接受健康体检的体检者100名为对照组。所有参与研究人员均接受尿常规定性检验,对比两组尿常规检验差异。并分析蛋白尿与糖尿病患者年龄及患病病程的相关性。结果 观察组患者的尿隐血、尿糖、白细胞脂酶、尿酮体、白细胞、尿蛋白阳性率均显著高于对照组,差异有统计学意义(P<0.05);观察组出现蛋白尿的患者中,一过性蛋白尿发生率显著高于持续性蛋白尿发生率;观察组患者的一过性蛋白尿发生率显著低于对照组,差异有统计学意义(P<0.05);观察组患者的持续性蛋白尿发生率显著高于对照组,差异有统计学意义(P<0.05);患者的病程越长,严重白蛋白尿发生率越高,说明糖尿病患病病程与蛋白尿之间的关系呈正比;患者的年龄越大,严重白蛋白尿发生率越高,说明糖尿病患病年龄与严重白蛋白尿之间的关系呈正比。结论 尿常规定性检验能够有效检出糖尿病患者的蛋白尿情况,能够对患者是否存在糖尿病肾病进行判断,临床应用价值显著。

Objective To investigate the application value of urine routine test in the diagnosis of diabetic nephropathy.Methods A total of 113 patients with diabetic nephropathy who were treated in Longyan First Hospital from January 2021 to April 2022 were selected as the observation group.Then 100 physical examination subjects who received health examination in the hospital during the same period were selected as the control group.All the participating researchers received the urine routine test,and the differences of the two groups of urine routine test were compared.The correlation between proteinuria and age and disease duration of diabetic patients was also analyzed.Results The positive rates of urine occult blood,urine glucose,leukocyte lipase,urine ketone bodies,leukocytes and urine protein in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).Among the patients with proteinuria in the observation group,the incidence of transient proteinuria was significantly higher than that of persistent proteinuria.The incidence of transient proteinuria in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of persistent proteinuria in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The longer the course of the disease,the higher the incidence of severe albuminuria,indicating that the relationship between the course of diabetes and proteinuria is proportional.The older the patient,the higher the incidence of severe albuminuria,indicating that the relationship between the age of diabetes onset and severe albuminuria was proportional.Conclusion Urine routine test can effectively detect proteinuria in diabetic patients,and can judge whether patients have diabetic nephropathy,which has significant clinical application value.
