摘要
目的:探讨局麻下微波消融术治疗子宫腺肌病的短期治疗效果,并探究与效果显著性的相关因素。方法:收集2020年6月—2021年12月就诊于中国医科大学附属盛京医院要求行保留子宫治疗的子宫腺肌病患者50例。符合纳入排除标准并及时随访的患者共计20例,进行局麻下微波消融治疗,并短期随访3月,统计患者痛经情况及子宫缩小率,并评估短期治疗效果,基于患者临床基本信息统计与治疗效果显著的相关因素。结果:微波消融治疗子宫腺肌病患者总有效率达90%,术后痛经评分较术前显著下降(P<0.05),术后子宫体积缩小(P<0.05)。BMI、内膜厚度、是否生产、是否进行过妇科手术、年龄对微波消融治疗子宫腺肌病短期疗效尚未见显著差异(P>0.05)。结论:子宫腺肌病在局麻下使用微波消融治疗后短期的临床效果显著,同时该治疗方法安全、微创,为希望保留子宫的患者提供了一种新的治疗思路。
Objective: To investigate the short-term therapeutic effect of microwave ablation under local anesthesia in the treatment of adenomyosis, and to explore the related factors with significant effect. Methods: A total of 50 patients with adenomyosis who desired to preserve the uterus in Shengjing Hospital of China Medical University from June 2020 to December2021 were collected. A total of 20 patients met the inclusion and exclusion criteria. All of them underwent microwave ablation under local anesthesia and were followed up for 3 months. The dysmenorrhea and uterine shrinkage rate of the patients were counted, and the short-term treatment effect was evaluated, based on the basic clinical information of the patients, and the related factors with significant treatment effect. Results: The total effective rate of microwave ablation in patients with adenomyosis was 90%. The postoperative dysmenorrhea score was significantly lower than that before operation(P<0.05), and the uterine volume was decreased after operation(P<0.05). There were no significant differences in the short-term efficacy of microwave ablation in the treatment of adenomyosis in terms of BMI, endometrial thickness, production, gynecological surgery,and age(P>0.05). Conclusion: The short-term clinical effect of adenomyosis of microwave ablation under local anesthesia is significant. The treatment method is safe and minimally invasive, which provides a new treatment way for patients who wish to preserve the uterus.
